ABSTRACT Objective The aim of this study is to identify and explore the unmet needs adolescents young adults living with endometriosis. Design An open‐ended online survey was conducted, questions derived from prior research looking at areas need in healthcare, career work, financial, information, psychological, social cultural domains. Setting Population Self‐selecting 18‐25 year olds surgically diagnosed endometriosis (self‐reported) currently Australia were included as participants. Methods Invitation participate an shared through media Australian organisations Royal Women's Hospital, Melbourne. Surveys analysed qualitatively template analysis. Main Outcome Measures Recording supportive care population carried out. Results One hundred thirty‐one respondents fit eligibility criteria being aged 18–25 years (median age 23 years). Most born (94%), university‐educated (54%) lived a metropolitan setting (69%). There range that presented across education, healthcare relationships. Group‐specific challenges identified: doctors either over‐ or underemphasising future fertility; disrupted sexual romantic life due painful sex; managing pain classroom workplace where periods are taboo; gender‐queer gynaecological medical spaces. Conclusions increasingly which patients receiving diagnosis precipitates shift patient care. treatment decisions made must be reflective unique who carry burden disease. Clinicians advised aware discuss their patients. Patient Public Contribution nine developed data preliminary series interviews tertiary women's centre. In asking these data‐informed community, broader sociocultural demographics disease states (including less symptomatic endometriosis) have provided understanding needs.

Endometriosis and inflammatory bowel disease (IBD) share some epidemiological, clinical pathogenetic features. A differential diagnosis between pelvic endometriosis IBD may be challenging, even for expert clinicians. In the present review, we aimed to summarize currently available data regarding relationship their possible association.

Two recent genomic studies have demonstrated that lower genetically-predicted testosterone is associated with higher risk of endometriosis. These results are concordant a suite additional findings, including: (1) links endometriosis and its correlates shorter anogenital distance (an indicator prenatal testosterone), (2) pain sensitivity under in humans animal models, (3) associations levels key endocrine endometriosis, including antimullerian hormone, luteinizing β-endorphin, (4) data from models showing development distances, earlier onset estrus, more-regular ovulatory cycles. Taken together, this evidence supports 'two-hit' model whereby low differentially programs the hypothalamic-pituitary-ovarian axis such way as to increase vulnerability, adult testosterone, combined early menarche, high sensitivity, other genetic environmental factors, leads increased rates high-risk women. Most importantly, these phenotypic findings suggest represents disorder well estrogen, indicate effects women urgently needed.

Background: There has been considerable progress in our understanding of endometriosis, but its pathophysiology remains uncertain. Uncovering the underlying mechanism rare instances endometriosis reported early postmenarcheal years and girls before menarche can have wide implications. Methods: We conducted a literature review all relevant articles on Medline. Results: In review, we explore pathogenetic theories premenarcheal role retrograde menstruation adult potential early-onset disease, as well factors that argue against existence link between (EOE) neonatal uterine bleeding (NUB). Conclusions: As with women, pathogenesis disease unclear. A NUB EOE is plausible, there are challenges to collating supporting evidence. The state development leaves many unknowns need exploration. These include proof viable endometrial cells or mesenchymal stem NUB, their passage pelvic cavity, possible response steroids, whether they reside within cavity remain dormant till menarche.
