Background The COVID-19 outbreak at the end of 2019 severely impacted global healthcare systems, especially primary services. This paper aimed to identify implications derived from strengths and weaknesses observed in Iran’s (PHC) programmes during pandemic. Methods was a qualitative study conducted 2021. 13 semistructured interviews were held with Iranian policymakers executive managers, selected via snowball sampling, using World Health Organization’s analytical framework. Finally, thematic analysis on interview data. Results findings yielded five major themes: revision financing, redefining education research healthcare, redefinition development new model for family medicine, community engagement. Conclusion Addressing vertical inequality system delineated be crucial. ABSTRACT Background Identifying transmission events that trigger epidemic spread is paramount for informing outbreak control. This study characterised the population diffusion patterns of SARS‐CoV‐2 across exposure settings and evaluate their ramifications in growth. Methods In Hong Kong, COVID‐19 clusters delineated through case‐based surveillance during pandemic period were classified into eight settings: residence, home gathering, neighbourhood, workplace (office)/school, (non‐office), daily activity, social activity healthcare. Diffusion by size, speed likelihood spillover (cases seeding a new cluster) compared among settings. With different emerging, lagged effect on effective reproduction number ( R t ) was evaluated.
