ABSTRACT Introduction Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have become increasingly prevalent and the potential to delay gastric emptying. The American Society of Anesthesiologists (ASA) released guidance regarding perioperative management patients receiving GLP-1 RAs, but it is unclear extent which hospitals in U.S. Military Health System implemented policies consistent with this guidance. Methods A questionnaire was sent active duty anesthesiologists Certified Registered Nurse Anesthetists (CRNAs) working System. questions assessed presence institutional RA policies, adherence policy (if applicable), basis components current recommended future tracking implementation outcomes, knowledge, skills, barriers, continuing medical education goals related RAs. Results response rate 32% (N = 265); a little over half respondents were (53%); respondents’ primary practices included 50 institutions. More than (54%) indicated that their practice location had policy; 65% whom they always followed policy. In review locations >1 respondents, there lack perfect agreement across most locations. commonly reported for ASA (87%), by department leadership (37%). Barriers any system-wide ultrasound comfort, as well skills pressure not cancel cases, scheduling problems, productivity requirements. Conclusions Formal inconsistencies within suggest local could be improved. Commonly barriers provide data-driven information efforts improve success.

Abstract Introduction: We define clinical practice guideline (CPGs) implementability as the characteristics of that reflect extent to which it is likely be adopted in practice. Our objectives were create a comprehensive and evidence-informed framework (CFGI). Methods:A mixed-methods approach was used. Based on systematic literature review six databases foundation, initial version CFGI created, followed by external consultations gather feedback natural language processing tool-assisted classificationto refine framework. To get validation from expert at an international conference Results: 15 studies related identified review. The first compiled, including 6 domains. Feedback received 16 stakeholders, clinicians, nurses, medical managers, methodologists, combined with classification. final comprised core domains, containing 21 constructs: (1) Scope purpose; (2) Clarity consistency recommendations; (3) Development evidence base; (4) Structure Contents; (5) team transparency; (6) Implementation environment tools. Twenty-nine experts participated validation, results showed had good rationality, importance, clarity, feasibility, necessity. Conclusions: development provides theoretical basis for implementation future CPGs, will help enhance guidelines facilitate their promotion application different settings. Future research can further validate apply CFGI, explore its effectiveness feasibility actual operation.
