We have innovatively amalgamated membrane blood purification and centrifugal cell separation technologies to address the limitations of current artificial liver support (ALS) models, develop a versatile plasma system (VPPS) through separation. To investigate influence VPPS on long-term rehospitalization mortality rates among patients with acute-on-chronic failure (ACLF). This real-world, prospective study recruited inpatients diagnosed ACLF from Second Xiangya Hospital Central South University between October 2021 March 2024. Patients were categorized into non-VPPS groups based distinct ALS models administered them. Self-administered questionnaires, clinical records, self-reported data served as primary methods for collection. The laboratory results evaluated at six time points. All subjected follow-up assessments > 12 months. Kaplan-Meier survival analyses Cox proportional hazards used evaluate risks hospitalization during period. A cohort 502 was recruited, 260 assigned group. On comparing baseline characteristics, group exhibited significantly shorter length stay, higher incidence spontaneous peritonitis pulmonary aspergillosis compared (P < 0.05). Age [hazard ratio (HR) = 1.142, 95%CI: 1.01-1.23, P 0.018), (HR 2.825, 1.07-6.382, 0.026), albumin 0.67, 0.46-0.942, 0.023), total bilirubin 1.26, 1.01-3.25, 0.021), international normalized 1.97, 1.21-2.908, 0.014), VPPS/non-VPPS 3.24, 2.152-4.76, 0.001) identified significant independent predictors in both univariate multivariate throughout demonstrated ≥ 2 years (log-rank test, 0.001). These findings suggest that is safe has positive prognostic outcomes ACLF.
