Abstract Background Body dysmorphic disorder (BDD) is particularly prevalent yet highly understudied and undertreated in adolescence. This study evaluates the efficacy of an internet-based, therapist-guided cognitive behavioral therapy (CBT) for adolescents young adults with BDD compared to supportive online as active control condition. Methods In a single-blind, randomized controlled trial, N = 45 (aged 15–21 years) all genders from German-speaking countries were assigned 12 sessions internet-based CBT (iCBT) or weeks therapy. The primary outcome was change expert-rated symptom severity pre- post-intervention (Yale-Brown Obsessive-Compulsive Scale Modified Dysmorphic Disorder, BDD-YBOCS). Secondary outcomes included remission responder rate, changes delusionality appearance beliefs (BABS), self-rated (FKS), cognitions (FKDK), quality life (KINDL-R), depressive symptoms (PHQ-9) pre post 4-week follow-up. Results iCBT more efficient than on BDD-YBOCS ( p =.002), large between-group effect size at (Hedges’ g SE ) 0.93 (0.42)), secondary measures <.05), except =.068). All also showed significant improvements iCBT, moderate sizes, gains stable until follow-up period. participants higher (61.5%) rates (66.7%), controls (0% 26.7%), but only difference reached significance. Conclusion results indicate comparison condition, thus contributing limited intervention research adolescent adding much-needed treatment option. Data collected at baseline, immediately post-intervention, three-month follow-up, analyzed using Analysis Variance Repeated Measurements Bonferroni post-hoc tests. led significant reduction rumination, mean scores decreasing 18.45 (SD = 3.52) pre-treatment 13.67 3.01) post-treatment, slightly rising 14.22 2.86) follow-up. Quality significantly improved, increasing 42.58 4.45) 49.73 4.12) slight follow-up 48.89 3.98). Statistical analysis confirmed time effects (F(2, 116) 19.45, p < 0.001, η² 0.15) × interaction 11.58, 0.10). findings suggest that is effective improving BDD. showed durability indicating its potential for long-term benefits. Body dysmorphic disorder (BDD) is severe and undertreated. Digital mental health could be key to expanding access evidence-based treatments, such as cognitive behavioral therapy for BDD (CBT-BDD). Coach guidance posited essential effective uptake of digital interventions. However, little known about how different patients may use coaching, what patterns correspond meaningful outcomes, match coaching patient needs. Participants were 77 adults who received a 12-week guided smartphone CBT-BDD. Bachelor's-level coaches available via asynchronous messaging. We analyzed the 400 messages sent by users during treatment. Message content was coded using efficiency model support (i.e., usability, engagement, fit, knowledge, implementation). aimed clarify when purposes with used if we can meaningfully classify these patterns. then assessed potential baseline predictors coach usage, whether distinct relate clinical outcomes. Users on average 5.88 (SD = 4.51, range 1–20) 9.84 5.74, 2–30). Regarding frequency sending messages, latent profile analysis revealed three profiles, characterized by: (1) peak mid-treatment (16.88 %), (2) bimodal/more communication early late in treatment (10.39 (3) consistent low/no (72.73 %). content, four profiles emerged, mostly engagement (51.95 fit (15.58 knowledge (4) miscellaneous/no There significant relationship between age, that early/late group older than low group, adherence, driven completing more modules contact group. groups began symptoms Content associated dropout, suggesting higher dropout rates reduced no membership other The majority participants initiated their most common function communications increase engagement. Results suggest individuals prefer or require younger counterparts Additionally, whereas primarily at lower risk dropping out, those do not engage all elevated risk.
