Background/Objectives: Liver transplantation is a life-saving procedure for patients with end-stage liver disease. In recent years, the demand has surpassed supply of available donor organs. Utilizing extended-criteria donors (ECDs) alleviates scarcity suitable livers transplantation. One ECD was history alcohol abuse. grafts from chronic and active abuse are typically promptly excluded, diminishing organ pool. This highlights need to re-evaluate exclusion criteria expand pool address ongoing shortage. Methods: We examined adult (>18 years) transplant recipients who received deceased had documented between 2011 2024. indications were conventional included hepatitis C virus (HCV), non-alcoholic steatohepatitis, alcoholic disease, disease coexisting HCV, cryptogenic cirrhosis, cholestatic primary biliary cholangitis, sclerosing metabolic hepatocellular carcinoma, hepatitis. compared 1-year, 5-year, 9-year survival rates those non-alcohol-consuming donors. Results: total, 370 included. At 1 year post-transplant, comparable two groups. Conclusions: yielded function outcomes non-alcohol-using By expanding include carefully screened alcohol-using donors, programs can improve access transplantations.
