Immune checkpoint inhibitor (ICI) treatment has dramatically revolutionized the landscape of therapeutic approaches in multiple cancers, particularly, non-small-cell lung cancer (NSCLC). With increasing use programmed death-1 (PD-1) inhibitors clinic, emerging toxicity profile presents a novel learning curve for clinicians. Here we report first case an NSCLC patient displaying sarcoid/granulomatous-like reaction (SLR, also known as GLR) liver during anti-PD-1 therapy which showed efficacious response complete regression. Also, this is describing SLR induced by toripalimab, PD-1 inhibitor. Given kind hepatic findings can be easily mistaken metastasis, even resulting premature second-line treatments. In particular, briefly review clinical features all those cases reporting sarcoidosis and SLRs manifested on different organs anti-PD-(L)1 therapy. We anticipate that these would help to alert attention clinicians SLRs, rare immune-related adverse event (irAE), manageable histopathological analysis necessary before interpreting it disease progression.

Abstract Background In patients with extrathoracic malignancies (ETM), granulomatous lymph adenopathy called sarcoid‐like reactions (SLR) can be seen in the regional or draining nodes. We hypothesized that SLR may a sign of imminent metastasis and investigated clinical course rate recurrence ETM mediastinal lymphadenopathy (MLN). Methods this retrospective observational study, we reviewed medical files known who underwent EBUS‐TBNA for initial staging detection from 2011 to 2023. Patients MLN were included. Results Forty‐one (29 female) enrolled study. Breast colorectal carcinomas most common malignancies. A total 81 nodes sampled. The final diagnosis was five sarcoidosis, one tuberculosis, second primary, drug reaction, 33 SLR. Among SLR, patient progressed during follow‐up accepted as false‐negative without confirmatory biopsy. negative predictive value (NPV) 97.05%. Conclusion Granulomatous due ETM. has high NPV Follow‐up imaging rather than biopsy is reasonable these patients.
