Abstract Background Type 2 diabetes (T2DM) and obesity are characterized by altered insulin metabolism action. Reduced hepatic clearance is increasingly recognized as a key contributor to hyperinsulinemia resistance. CEACAM1 promotes clearance, its loss in hepatocytes associated with reduced mice men. This study examines whether circulating levels reflect compromised resistance the PREVADIAB2 cohort. Methods A total of 1019 individuals from cohort were evaluated for 75 g‐OGTT classified according WHO 2019 criteria. measured ELISA, parameters calculated. Hierarchical clustering metabolic indices was performed. Statistical significance assessed using Kruskal–Wallis Wilcoxon–Mann–Whitney tests. Results BMI, (HOMA‐IR), steatosis progressively increased disease severity. Insulin secretion rose declined parallel prediabetes T2DM, indicating compensatory hyperinsulinemia. identified four clusters distinct patterns further showed that positively correlated CEACAM1, especially normoglycemia, lower steatosis. suggests can status clearance. Conclusions demonstrates progressive increase elevated BMI prevalence, accompanied declining levels. Cluster analysis linked levels, suggesting potential usefulness biomarker progression.
