The thalamus plays an essential role in cognition. Cognitive deficits have to date mostly been studied retrospectively chronic thalamic stroke small cohorts. Studies prospectively evaluating the evolution of cognitive and their association with topography are lacking. This knowledge is relevant for targeted patient diagnostics rehabilitation.Thirty-seven patients (57.5±17.5 [mean±SD] years, 57% men) first-ever acute isolated ischemic covering anterior (n=5), paramedian (n=12), or inferolateral (n=20) 37 in-patient controls without similar vascular risk factors matched age sex were studied. Cognition was evaluated using predefined tests at 1, 6, 12, 24 months. Voxel-based lesion-symptom mapping used determine associations between neuropsychological topography.Patients revealed severe verbal memory (median T score [Q1-Q3]: 39.1 [36.1-44.1]), language (31.8 [31.0-43.8]), executive functions (43.8 [35.5-48.1]) 1 month compared (verbal memory: 48.5 [43.6-61.0], language: 55.7 [42.3-61.1], functions: 51.3 [50.1-56.8]). Patients showed moderate (44.7 [42.8-55.9]) (49.5 [44.3-55.1]) no (48.1 [42.5-54.7]) (59.0 [47.0-64.5]; 59.6 [51.1-61.3]; 52.5 [44.2-55.3]). almost completely recovered during follow-up. Intriguingly, significant [41.5-51.9]), (47.5 [41.8-54.1]), (48.2 [46.2-59.7]) found (50.5 [46.7-59.9]; 57.0 [51.2-62.9]; 57.4 [51.2-60.7]). Language, but not persisted lesions non-verbal memory, language, anterior/paramedian/inferolateral border. All 3 topographies exhibited diverse domains, which a different degree depending on localization. Our study emphasizes need comprehensive secure adequate rehabilitation.

Background: Post-stroke cognitive impairment (PSCI) is a major complication of stroke that affects more than one-third survivors, threatening their quality life and increasing the risk disability death. Although various studies have described etiology, epidemiology, factors PSCI, there are limited number comprehensive accurate reports on research trends hotspots in this field. Therefore, review aimed to evaluate trends, hotspots, frontiers PSCI using bibliometric analysis. Methods: We screened literature spanning 20 years Web Science Core Collection: Citation Index Expanded (SCI-Expanded) database from 1 January 2003 31 December 2022. included all eligible based our search strategy, inclusion criteria, exclusion criteria. The analysis annual publications, countries/regions, institutions, journals, co-cited references, keywords was conducted CiteSpace VOSviewer, findings were summarized. Results: A total 1,024 publications review. found increased annually. These published 75 countries or regions by over 400 institutions. Chinese institutions had highest international influence limited. United States showed strong journal “Stroke” most (57) with high impact factor considered journal. frequently cited references focused prevalence, incidence, neuropsychological assessment scales, guidelines PSCI. strongest citation burst for “neurotrophic factor” “synaptic plasticity”, which regarded as focuses respectively. Conclusion: This provided summary identified authoritative clarified research, highlighted Currently, mechanisms treatment limited, we hope has effectively trajectory will lay foundation innovative future.
