歯周炎は糖尿病と密接な関連があり,糖尿病合併症のような側面が知られている。主な糖尿病の慢性合併症ではインスリン抵抗性の発現が重要な要因となっている。歯周組織においても同様に,前糖尿病段階から血管内皮細胞に炎症が起き,歯肉にはインスリン抵抗性が生じていることが見出された。そして,高血糖による活性酸素種(ROS)産生などの酸化ストレスがインスリン抵抗性発現に関与し,細胞機能の低下や歯肉の創傷治癒遅延,歯周組織再生の悪化,インプラントのオッセオインテグレーションの悪化に関わっていることが示された。

Background: Periodontitis is a chronic infectious–inflammatory pathology, with high prevalence, which destroys the dental support and, if left untreated, leads to tooth loss. Objectives: Our objective was conduct review of systematic reviews meta-analyses determine evidence for periodontal treatment on periodontitis and diabetes. Second, we assessed risk bias methodological quality using AMSTAR-2 ROBIS tools. Methods: We performed bibliographic searches in PubMed/Medline, Embase, Cochrane Central, Dentistry & Oral Sciences Source databases Web Science (WOS) scientific information service identify from last five years. Results: Eighteen studies that met inclusion criteria evaluated 16,247 subjects were included. The most studied parameters probing pocket depth, clinical attachment level, bleeding glycated hemoglobin.
