Background: While studies on cervical cancer screening (CCS) and human papillomavirus (HPV) vaccine uptake exist in China, few have examined both within populations participating CCS programs. Methods: This cross-sectional, population-based survey included 37,797 women aged 18– 64 years attending gynecological outpatient clinics. Participants completed a questionnaire assessing awareness participation. Additionally, HPV knowledge was investigated sub-sample of 17,010 < 45 years. Results: The mean age participants 45.06 (SD: 8.52). score 63.13%, with 52.53% reporting prior uptake. Factors associated lower low monthly household income, lack rural cooperative medical insurance, literacy, not having received the past 3 years, unmarried status, childbirth history. A strong positive correlation observed between adherence to recommendations (Spearman coefficient: 0.708, p 0.01). Awareness reported by 48.33% participants, positively correlated 0.629, partially mediated relationship willingness vaccinate, mediating effect size 21.16%. Conclusion: Integrated health education efforts targeting are essential. Empowering understand significance vaccination regular participation can enhance prevention strategies.
