Abstract Background Despite data supporting the safety of SARS‐CoV‐2 vaccination, concerns regarding receipt blood products from donors previously infected or vaccinated against persist. We assessed whether transfusions plasma platelet with prior infection vaccination were associated adverse outcomes in patients without COVID‐19. Methods linked donor spike and nucleocapsid antibody history to transfused between June 1, 2020 March 31, 2022. used logistic regression, adjusting for demographics comorbidities, calculate odds ratios 95% confidence intervals (CI) posttransfusion thrombosis, increased respiratory requirement, hospital mortality. Outcomes as per unit compared units uninfected unvaccinated donors. Results Among 8715 hospitalizations 7773 transfusion recipients data, there 251 thromboses, 700 requirements, 1443 deaths. 15,167 units, 4993 1106 donors, respectively. 19,295 8530 1368 There no associations mortality (all CI including 1). Nor recently high titers outcomes. Discussion Donor SARS‐Cov‐2 not patient do need be considered allocation.

Abstract Background The Association for the Advancement of Blood and Biotherapies guidelines recommend use high‐titer COVID‐19 convalescent plasma (CCP) patients with SARS‐CoV‐2 at high risk disease progression, including those who are immunocompromised. We hypothesized that conventional units have comparable neutralizing antibody levels to CCP. Study Design Methods Conventional CCP were obtained from blood suppliers. Quantitatively measured antibodies assessed using MesoScale Discovery multiplex electrochemiluminescence immunoassay. Binding distributions compared Wilcoxon rank‐sum tests. analyzed GeneScript ELISA‐based neutralization assay. proportion a percent signal inhibition ≥80% (as defined by United States Food Drug Administration in 2021) exact binomial confidence intervals (CIs) calculated. Results Among 218 74 collected between September 2023 July 2024, distribution total binding largely overlapped CCP, though statistically significant differences median nucleocapsid spike Omicron variant concentrations observed. Median was 97.5% (range 3.4%–98.6%) among 97.7% 95.4%–98.6%) units. For plasma, 95.0% (95% CI = 91.2%–97.5%) met threshold 100% 95.1%–100.0%) Conclusion demonstrate similar concentration In countries or regions where licensed is unavailable titers unknown, transfusion multiple may be clinical utility.

BACKGROUND: The VIPS (Vascular Effects of Infection in Pediatric Stroke) II prospective cohort study aimed to better understand published findings that common acute infections, particularly respiratory viruses, can trigger childhood arterial ischemic stroke (AIS). COVID-19 pandemic developed midway through enrollment, creating an opportunity assess its impact. METHODS: Twenty-two sites (North America, Australia) prospectively enrolled 205 children (aged 28 days 18 years) with AIS from December 2016 January 2022, including 100 cases during the epoch, defined here as 2020 2022. To background rates subclinical infection, we stroke-free well children, 39 pandemic. We measured serum SARS-CoV-2 nucleocapsid total antibodies (present after not vaccination; half-life 3–6 months). assessed clinical infection via parental interview. RESULTS: monthly rate eligible declined spring fall 2020, recovering early 2021 and peaking spring. prepandemic were similar except had fewer infections prior month (17% versus 30%; P =0.02) more focal cerebral arteriopathy (20% 11%; =0.09). Among cases, 26 (26%) positive antibodies, 4 (10%) pandemic-era ( =0.04). first case occurred July 2020. Ten (38%) a recent by report, 7 those 10 received diagnosis COVID-19. Only 1 multisystem inflammatory syndrome children. Median (interquartile range) IgG levels 50.1 S/CO (specimen calibrator absorbance ratio; 26.9–95.3) 18.8 (12.0–101) =0.33). CONCLUSIONS: may have dual effects on AIS: indirect protective effect related public health measures reducing infectious exposure general, deleterious emerged another virus AIS.
