ABSTRACT Cardiac computed tomography angiography (CCTA) has acquired a pivotal role in modern cardiology. It represents the gold standard for noninvasive coronary imaging. Moreover, CCTA permits comprehensive evaluation of atheromatic burden and plaque composition. This study aims to review impact across different presentations ischemic heart disease, from primary prevention patients requiring revascularization.

Impaired glucose tolerance (IGT), a prediabetic state, is known risk factor for coronary artery disease (CAD). Low-attenuation plaque (LAP) lesions are associated with high of events. We aimed to evaluate high-risk characteristics in LAP between patients IGT and normal (NGT) suspected stable CAD. Coronary computed tomography angiography (CCTA) identified assessed volumes, burdens, features. Glycemic was stratified using oral tests. Among 148 patients, 202 were identified, 93 classified as NGT 55 IGT. Patients had significantly higher prevalence compared (p = 0.007). volume (16.46 ± 12.52 mm3) (12.66 9.72 mm3, p 0.01), but this association did not persist multivariate analysis. The burden greater (10.79 6.84%) than (8.62 5.93%, 0.02), the napkin-ring sign more frequent (12%) versus (5%, 0.02); these associations remained significant frequency signs. These findings may help explain common occurrence prediabetes acute myocardial infarction.

Plaque progression (PP) is critical between subclinical atherosclerosis and plaque rupture. Small dense low-density lipoprotein cholesterol (sdLDL-C) considered as the most atherogenic lipoprotein. This study aims to investigate relationship sdLDL-C level PP in patients with stable coronary artery disease. We conducted a retrospective analysis of 146 lesions 86 by repeat intravascular ultrasound examinations from January 2020 May 2023. was determined increases percent atheroma volume, defined volume proportion occupied entire vascular wall, ≥5% during follow-up. Time-averaged values were calculated for all cardiometabolic parameters including sdLDL-C. Multivariate logistic regression performed interrogate association time-averaged PP. During median follow-up 12.6 months, found 65 (44.5%), mean changes 4.1%±10.2%. A positive correlation observed total (Pearson r=0.29, P=0.006), especially diabetic r=0.58, P<0.001). After multivariate adjustment, every 0.1-mmol/L increase conferred 1.2-fold increased risk Our findings suggest that an independent factor Intensive control along other factors should be mitigate improve cardiovascular outcomes.

Through complex pathways that continue to be elucidated, hypertension damages tissues, organs and systems, has become a public health risk factor disease of global importance. Its mechanisms, complications on organ their salient features, are outlined highlight how they could understood inform contemplating clinical, policy strategies. Among these are: cardiac including coronary artery atherosclerosis, myocardial ischaemia hypertrophy its complications; cerebrovascular haemorrhagic ischaemic stroke; renal leading end stage failure; vascular aortic aneurysm diffuse peripheral arterial disease; wide range ocular complete permanent vision loss. Ultimately, premature death can complicate untreated hypertension. More is needed implement as the issue it address consequences.

The aim of this study was to evaluate the differences in plaque composition and burden between normal glycemic status (NGS) dysglycemia expressed as impaired glucose tolerance (IGT) type 2 diabetes mellitus (T2DM). Clinically indicated coronary computed tomography angiography used patients with suspected artery disease (CAD). An oral test performed assess status. Patients were stratified NGS, IGT, T2DM. Plaque volumes quantified using validated software, further compositional measurements low-attenuation, non-calcified, calcified burden. Of 355 CAD, 220 had 92 diagnosed 43 known Low-attenuation volume significantly higher IGT (209 mm3, p < 0.02) T2DM (243 = 0.005) compared NGS (166 mm3). Total similar all groups, but a greater low-attenuation seen (p 0.03) NGS. multivariate linear regression model adjusted for clinical risk factors showed that those 0.03). Interestingly, no significant burdens observed both univariate analyses. Dysglycemia, including mellitus, associated increased demonstrated T2DM, underscoring need early metabolic intervention.
