Introduction: Stroke is a leading global contributor to mortality and disability. Low‐ middle‐income countries are disproportionately affected account for 87% of stroke‐related disabilities 70% fatalities. The challenges stroke care accessibility in Africa compounded by financial constraints, geographical barriers, inadequate healthcare infrastructure, necessitating the adoption innovative models such as telestroke. Conclusion: has demonstrated significant benefits including improved reduced onset‐to‐treatment time, enhanced diagnostic accuracy, increased accessibility, particularly medically regions. Background Principal Illness Navigation (PIN) services may play an important role in helping patients through transitions care following acute hospitalization. We evaluated a novel PIN telemedicine approach to understand the feasibility of providing these diverse patient cohorts. Methods A single-arm, retrospective observational study Kandu Health’s post-acute service was conducted experiencing ischemic or hemorrhagic stroke California and New Jersey. The technology-enabled program offered remote healthcare support led by occupational therapists licensed clinical social workers that tailored individual needs facilitate transition community settings post-discharge. Kebutuhan public awareness atau kesadaran masyarakat akan pentingnya pengenal terhadap signs dinilai sangat kritikal penting mengatasi kesenjangan informasi dimasyarakat terutama di masa pandemi COVID-19. Tujuan utama PKM umum pencegahan Stroke mengenali tanda gejala awal penerapan gaya hidup sehat dalam Pandemi. Kegiatan dilakukan daring ZOOM Meeting menjabarkan materi terkait Mengenali Warning Signs Stroke. Target kegiatan Pengabdian Masyarakat sebanyak 86 terjangkau oleh kelompok flyer disebarkan media sosial. Berdasarkan hasil Pre-Post Test terdapat peningkatan nilai rerata 20.6 poin penilaian Pre-Test 68.3 Post-Test 88.9 poin.
