Ischemic stroke is a fatal and disabling disease worldwide imposes significant burden on society. At present, biological markers that can be conveniently measured in body fluids are lacking for the diagnosis of ischemic stroke, there no effective treatment methods to improve neurological function after stroke. Therefore, new ways diagnosing treating urgently needed. The neurovascular unit, composed neurons, astrocytes, microglia, other components, plays crucial role onset progression Extracellular vesicles nanoscale lipid bilayer secreted by various cells. key extracellular vesicles, which released cells unit serve as facilitators cellular communication, has been extensively documented recent literature. Here, we highlight unit-derived current status vesicle engineering treatment, problems encountered clinical translation therapies. derived from could provide an important contribution diagnostic therapeutic tools future, more studies this area should carried out.

Acute brain injuries (ABI) caused by various emergencies can lead to structural and functional damage tissue. Common causes include traumatic injury, cerebral hemorrhage, ischemic stroke, heat stroke. Globally, ABI represent a significant portion of neurosurgical cases. Previous studies have emphasized the therapeutic potential stem cell-derived extracellular vesicles (EVs). Recent research indicates that EVs extracted from resident cells in central nervous system (CNS) also show following injury. Microglia, as innate immune CNS, respond changes internal environment altering their phenotype secreting impact CNS cells, including neurons, astrocytes, oligodendrocytes, endothelial neural (NSCs), microglia themselves. Notably, under different external stimuli, either promote neuronal survival, angiogenesis, myelin regeneration while reducing glial scarring inflammation, or they exert opposite effects. This review summarizes evaluates current findings on how microglia-derived influence after stimuli. It analyzes interaction mechanisms between discusses future directions clinical applications.

Neuropathic pain (NP) is a common symptom of many diseases and caused by direct or indirect damage to the nervous system. Tricyclic antidepressants serotonin-norepinephrine reuptake inhibitors are typical drugs used in clinical practice suppress pain. However, these have drawbacks, including short duration action, limited analgesic effect, possible dependence side effects. Therefore, developing more effective NP treatment strategies has become priority medical research attracted much attention. P2X7 receptor (P2X7R) non-selective cation channel activated adenosine triphosphate mainly expressed microglia central Microglial P2X7R plays an important role regulation, suggesting that it could be potential target for drug development. This review comprehensively objectively discussed latest progress P2X7R, its structural characteristics, functional properties, relationship with microglial activation polarization, mechanism therapeutic multiple models. study aimed provide in-depth insights into association between explore action pathological process translational application prospects antagonists treatment, providing scientific basis precise NP.

ApTOLL, a promising therapeutic agent, has demonstrated effectiveness in mitigating stroke-induced neurological damage clinical settings. Despite this, the detailed molecular mechanisms by which ApTOLL impacts ischemic stroke remain inadequately understood. In present study, we explored how modulates downstream microRNAs (miRNAs) to alleviate brain and cognitive dysfunction associated with stroke. We established rat model of Administration upregulated miR-335-5p suppressed IRAK1 expression brain. treatment significantly reduced infarct size, diminished neuronal apoptosis, attenuated pathological Additionally, led inhibition inflammation oxidative while enhancing autophagy. Similar effects were observed when was overexpressed or knocked down. Conversely, beneficial negated antagomir overexpression, suggesting that ApTOLL's neuroprotective are mediated miR-335-5p/IRAK1 pathway. Mechanistically, exerted its protective role promoting miR-335-5p, thereby reducing levels, leading amelioration damage. effectively mitigates modulating axis.
