Pediatric epilepsy presents challenges in treatment optimization, with a significant proportion of patients experiencing inadequate seizure control despite anti-seizure medications (ASMs) therapy. Recent research has indicated the involvement neuroinflammation and immune-mediated mechanisms pathogenesis, suggesting potential interplay between immunological factors ASMs responsiveness. This study aimed to investigate role pediatric generalized, focal their interaction understand influence on outcomes. A retrospective cohort was conducted involving 136 patients, categorized into Anti-seizure Insensitive Group (n = 67) Sensitive 69). Immunoglobulin levels factors, including cytokines, were assessed before treatment. Seizure characteristics also analyzed. Associations characteristics, sensitivity evaluated. The revealed differences interleukin-6 (IL-6), IL-1β IL-10 levels, insensitive sensitive groups. Furthermore, frequency, drug-resistant seizures, severity, seizure-free period, status epilepticus all demonstrated correlations ASMs, negative for positive period epilepticus. highlights complex immune function, mechanisms, underscoring need comprehensive understanding modulation drug response epilepsy. Cannabidiol (CBD) is one of the most prominent non-psychotropic cannabinoids with known therapeutic potentials. Based on its anti-seizure efficacy, first cannabis derived pharmaceutical grade CBD-based medication was approved in USA 2018 for treatment seizures patients 2 years and older. Despite effectiveness reducing seizures, there remain several major questions optimization CBD therapy epilepsy such as optimal dosage, composition, route delivery, which are main objective this current study. We evaluated antiseizure effects through different compositions, routes dosages a pre-clinical model. used kainic acid-induced model C57BL/6 mice, treated them placebo and/or inhalation, oral, injection (intraperitoneal) routes. broad spectrum (inhaled intraperitoneal) vs isolate formulations. employed Racine scaling system to evaluate severity flow cytometry measuring immune biomarkers neurotrophic factors, histologic analysis examine compare groups. Our findings showed that all forms reduced severity. Among combination tested, via inhalation effective epileptic (p < 0.05) compared other treatments. data suggest formulations affect efficacy prevention seizures. Inhaled potential superior effect delivery warrants further research.

Gliomas and brain metastases are notorious for their dismal prognosis low survival rates, a challenge exacerbated by our incomplete grasp of the complex dynamics that govern cancers. Recently, groundbreaking paradigm shift has emerged, highlighting crucial role synaptic communication between neurons tumor cells in reshaping neuronal signaling to favor growth. This review delves into pivotal interplay mechanisms, focusing on excitatory glutamatergic inhibitory GABAergic pathways. Glutamatergic synapses utilize glutamate propagate signals, while employ gamma-aminobutyric acid (GABA) inhibit firing. can be broadly classified ionotropic (NMDAR, AMPAR kainite receptors) metabotropic subtypes. The harmonious orchestration these types is essential normal function, dysregulation implicated neurodegenerative disorders such as Alzheimer's disease epilepsy. Emerging evidence reveals glioma metastatic exploit pathways neurotransmitters enhance proliferation survival. In this review, we will first explore intricate mechanisms underlying signaling. Next, summarize recent advancements understanding how cancer hijack advantage. Finally, propose novel therapeutic strategies aimed at disrupting aberrant neuron-tumor communication, offering potential treatment combating otherwise incurable

Refractory epilepsy, characterized by seizures that do not respond to standard antiseizure medications, remains a significant clinical challenge. The central role of the immune system on occurrence epileptic disorders has been long studied, but recent perspectives immunometabolism and neuroinflammation are reshaping scientific knowledge. ketogenic diet its variants have considered an important medical nutrition therapy for refractory epilepsy may potential modulation effect system, specifically, metabolism cells. In this comprehensive review, we gathered current evidence-based practice, interventional ongoing trials addressing in epilepsy. We also discussed detail anticonvulsant mechanisms, neuroimmunometabolism, highlighting Th17/Treg homeostasis as one most interesting aspects deserving consideration future trials.
