Background: Paramedics are increasingly involved in palliative care and often support community-based service delivery to facilitate integrated practice. However, the impact of specific guidelines on clinical practice remains unknown. Aim: To determine an ambulance guideline rates supportive medication administration non-transport. Design: A retrospective cohort study electronic patient records from January 2014 June 2023. Baseline characteristics were compared pre- post-guideline introduction. Interrupted time series analysis was performed examine efficacy. Setting/participants: Patients all ages receiving who attended by paramedics Victoria, Australia. Results: total 31,579 patients included. The median age 75 years (IQR = 64–84 years), 56.4% men. Overall, 25.8% not transported hospital. Following introduction, there no significant trend changes medications. non-transport rate increased significantly per month (0.2%, p 0.007), amounting a 9.9% ( 0.020) increase end period scenario which had been introduced. Subgroup diagnosed with ‘pain’ or after-hours also showed increases (monthly increase: pain 0.3%, 0.003; < 0.001; 29.7%, 0.001, 22.6%, 0.001). Conclusions: Introduction associated decrease transport emergency departments, allowing more continuity community.
