Published: April 5, 2024

BACKGROUND Surgical recovery after hospital discharge often presents challenges for patients and caregivers. Postoperative complications poorly managed pain at home can lead to unexpected visits the emergency department and/or readmission hospital. Digital monitoring (DHM) may improve postoperative care compared standard methods. OBJECTIVE We conducted a feasibility study randomized controlled trial (RCT) assess DHM's effectiveness following thoracic surgical procedures care. METHODS two-arm parallel-group pilot RCT single tertiary center. Adult undergoing were 1:1 into two groups: digital (DHM group) of (control group). adhered intention-to-treat analysis principle. The primary outcome was predetermined criteria. would be feasible if more than 75% recruitment, protocol adherence, data collection achieved. Secondary outcomes included 30-day visit rates, complications, length stay, post-discharge opioid consumption, quality (QOR-40), patient/program satisfaction, caregiver healthcare provider satisfaction cost-per-case. RESULTS All criteria met. recruitment rate 87.9% (95% CI: 79.4% – 93.8%). Protocol adherence rates 96.3% 89.4% - 99.2%) 98.7% 92.9% 99.9%), respectively. Eighty randomized, with 40 (50%) in DHM group control group. Baseline patient clinical characteristics comparable between groups. had fewer unplanned ED (2.7% vs. 20.5%; p=.02), admission (0% 7.6%; p=.24), lower (20% 47.5%, p=.01) shorter stays (4.0 6.9 days; p=.05), but consumption (111.6 ± 110.9 74.3 71.9 mg morphine equivalents; p=.08) also resulted times (130 0 1048 + 1093 minutes; p=.48) cost per case ($12,145 8,779 $17,247 15,313; p=.07). scores clinically significantly better controls (185.4 2.6 178.3 3.3; p<.0001). 37 who completed intervention answered program survey questionnaires (100%; 95% 90.5% 100%). Only 36 out 80 caregivers responded end 4th week post-hospital (47.7%; 35.7% 59.1%). Healthcare providers reported 100% rate. CONCLUSIONS This demonstrates conducting full-scale efficacy improving surgery. shows promise enhancing continuity warrants further investigation.
