Adipokines have not been studied in acute dengue, despite their emerging role inducing and regulating inflammation. Therefore, we sought to identify adipokine levels patients with varying severities of dengue understand disease pathogenesis. We determined the leptin, resistin, omentin, adiponectin, as well IFNβ, NS1 using quantitative ELISA fever (DF=49) haemorrhagic (DHF=22) at admission (febrile phase) time discharge (recovery phase). The viral loads serotypes all samples were quantified real-time RT-PCR. Resistin (p =0.04) omentin (p=0.006) significantly higher who developed DHF. Omentin febrile phase also correlated AST (Spearman's r=0.38, p=0.001) ALT r=0.24, p=0.04); serum leptin both r=0.27, p=0.02) r=0.28, p=0.02). Serum adiponectin did correlate any other adipokines or liver enzymes, but inversely CRP r=-0.31, p=0.008). Although significant (p=0.14) IFNβ lower those progressed develop DHF (median 0, IQR 0 39.4 pg/ml), compared had DF 37.1, 65.6 pg.ml). data suggest that are likely play a pathogenesis which should be further explored for potential used prognostic markers therapeutic targets.

Abstract Background Early detection of dengue fever is pivotal to help differentiate against other febrile illnesses, especially in the dengue-endemic regions. Currently, patients would have go healthcare facility get tested for dengue. A diagnostic approach that empowers persons perform their own tests needed. Here, we evaluated utility and usability NS1 rapid test whether it could be accepted as a home self-test. lateral flow immunochromatography (ICT) using DENV was converted possible self-test kit by providing with device, an alcohol swab, lancet, disposable dropper, assay buffer, manual. Methods Twenty volunteers were recruited this study. The participants performed under observation trained observers who recorded if any procedural error committed. results each interpreted given interpretation table. Results Ninety-five percent (95%) study found instruction manual easy follow 70% felt use. Additionally, 80% successfully obtained expected results. majority (80%) consider needed recommend family members friends. Seventy (70%) participants, most whom monthly income less than USD417, willing pay USD2 kit. Conclusion Findings from suggested highly acceptable and, hence, viable early infection. Since its discovery, vitamin D (VD) has been known for implications in maintaining bone homeostasis. However, recent years it discovered that the receptor is expressed on different cells of immune system and these can locally produce active form this molecule, calcitriol, strongly suggesting might play a key role both branches system, innate adaptive. Recent evidence demonstrated VD participates protective phases against invading microorganisms, including activation production antimicrobial peptides, inactivation replication infectious agents, prevention exposure cellular receptors to microbial adhesion, and, more importantly, modulation inflammatory response. In years, world witnessed major outbreaks an ancient disease, dengue fever; emergence pandemic caused by unknown virus, SARS-CoV-2; resurgence common respiratory infection, influenza. Despite belonging viral families, etiological agents infections present trait: their capacity cause complications not only through cytopathic effect target tissues but also excessive response produced human host infection. Abstract Background: Cardiovascular diseases (CVD) are the leading cause of death in South Asia with visceral adiposity and sarcopenic obesity emerging as critical risk factors. Methods: We conducted a cross-sectional study 139 Sri Lankan adults (mean age 39.3, SD ± 10.7, 52 females 87 males) to assess adipose tissue (VAT) area, fat mass index (FMI), ratio (FMR) lean muscle mass, expressed appendicular (ALMI) using dual energy Xray absorptiometry (DEXA). Fasting blood sugar (FBS, lipid profiles, apolipoproteins (A1 B) inflammatory markers (IL-6 IL-1b) were analysed. Results: 23 (44%) 67 (77%) males had VAT area ≥ 100cm2, 34 (65%) 79 (91%) an FMI above cut-off value 30 (58%) females, 33 (38%) FMR value. Females high (≥100cm2) significantly higher levels FBS (p=0.008) lower HDL Apo A1 (p<0.05) compared <100cm2. Further, ≥9 (p=0.01), those 1.2 ApoB ApoA1 levels. There no differences metabolic factors or mediators (≥6) FMR. low ALMI below (<5.5), while 47 (54%) (<7). levels, ApoB/ApoA1 serum cholesterol <7, than normal ALMI. Conclusion: Visceral appear be prevalent Asians gender specific implications. Given marked rise CVD, diabetes related Asia, urgent measures should adopted reduce burden illness due these illnesses associated frailty.

Abstract Background As many studies have shown conflicting results regarding the extent of viraemia and clinical disease severity, we sought to investigate if during early dengue illness is associated with subsequent severity. Methods Realtime PCR was carried out identify virus (DENV serotype), in 362 patients, presenting within first 4 days illness, from 2017 2022, Colombo Sri Lanka. To characterize all patients were followed throughout their daily severity classified according WHO 1997 2009 classification. Results 263 had DF, 99 progressed develop DHF, 15/99 DHF developed shock (DSS). Although viral loads higher febrile phase who than DF this not significant ( p = 0.5). Significant differences observed infected different DENV serotypes 0.0009), lowest detected DENV2 highest DENV3. Sub-analysis for association each serotype again significant. those lower loads, infection significantly a risk developing 0.011, Odds ratio 1.9; 95% CI 1.164 3.078). Based on classification, 233 warning signs (DWW), 114 without (DWoWS), 15 severe (SD). No difference between SD, DWW DWoWS 0.27). Conclusions Viral serotypes, do appear associate large Lankan cohort.

The COVID-19 pandemic caused by SARS-CoV-2 spread across many countries between 2020 and 2022. similarities in clinical presentation with other endemic diseases pose a challenge to physicians effectively diagnosing treating the infection. Approximately 129 nations have risk of dengue infection, more than 100 those are dengue. During pandemic, number cases decreased owing isolation measures followed. However, common them has led misdiagnosis. Both fever cause surge pro-inflammatory cytokines chemokines, thus sharing pathophysiology. False positive serological test results also posed difficulty differentiating fever. This review aims compare features, pathophysiology, immune response COVID-19, benefit public health management during pandemic.

The Coronavirus Disease 2019 (COVID-19) and dengue fever (DF) pandemics both remain to be significant public health concerns in the foreseeable future. Anti-SARS-CoV-2 drugs vaccines are indispensable eliminate epidemic situation. Here, two piperazine-based polyphenol derivatives DF-47 DF-51 were identified as potential inhibitors directly blocking active site of SARS-CoV-2 DENV RdRp. Data through RdRp inhibition screening an in-house library vitro antiviral study selected effective SARS-CoV-2/DENV polymerase. Moreover, silico simulation revealed stable binding modes between DF-47/DF-51 RdRp, respectively, including chelating with Mg2+ near polymerase site. This work discovered inhibitory effect polyphenols on distinct viral which expected developed into broad-spectrum, non-nucleoside new scaffold.
