Background Accurate distinction between stroke etiologic subtypes is critical for physicians to provide tailored treatment. The triglyceride-glucose (TyG) index, a marker of insulin resistance, has been associated with risk but its role in distinguishing remains unclear. We aimed assess the TyG index's ability differentiate cardioembolic (CE) from non-cardioembolic (NCE) strokes.

The triglyceride-glucose (TyG) index, renowned for its efficacy and convenience in assessing insulin resistance, has been validated as a reliable indicator various cardiovascular conditions. current study aims clarifying the link of TyG with incidences prognoses cardiac arrest (CA) following acute myocardial infarction (AMI). Our analysis is multicenter, retrospective utilizing data from Medical Information Mart Intensive Care IV eICU Collaborative Research Database. Patients AMI whom could be calculated within first 24 h after admission were included. main endpoints in-hospital ICU mortalities. Correlations between outcomes evaluated using logistic regression models, restricted cubic splines (RCS), well correlation linear analyses. Overlap weighting (OW), inverse probability treatment (IPTW), propensity score matching (PSM) methodologies utilized to balance cohorts, thereby minimizing potential biases. Subgroup analyses performed accordance identified modifiers. In total, 5208 individuals diagnosed AMI, among 371 developed CA, ultimately Higher levels observed populations CA compared those without [9.2 (8.7–9.7) vs. 9.0 (8.5–9.4)], demonstrated moderate discriminatory capacity identifying occurrences entire populations. Multivariate regressions revealed serves significant risk both (OR 1.711) mortalities 1.520) AMI-CA patients, it also associated prolonged LOSs. RCS confirmed relationships ascending increased mortality risks (P nonlinearity: 0.592 0.816, respectively), which persisted PSM, OW, IPTW adjustments. further strong rates elders, females, BMI < 28 kg/m2, hypertension. Elevated found apparently correlate higher prevalence adverse regarding patients AMI. findings point fresh insight into significance critically ill coronary

The role of the triglyceride-glucose (TyG) index in critically ill patients with congestive heart failure (CHF) and atrial fibrillation (AF), requiring intensive care unit (ICU) admission, remains unclear. This study aimed to investigate association between TyG clinical prognosis CHF AF. retrospective observational cohort utilized data from Medical Information Mart for Intensive Care-IV (MIMIC IV2.2) database. Participants were categorized into four groups based on level. primary outcome was hospital all-cause mortality. Multivariable logistic proportional regression analysis restricted cubic spline employed assess index's mortality Sensitivity included determining feature importance through subgroup different subgroups. A total 787 study, ICU mortalities 14.2% 8.3%, respectively. Multivariate demonstrated that independently associated an increased risk (odds ratio (OR), 1.59 [95% confidence interval (CI) 1.15-2.19], P = 0.005) [OR 1.9; (95% CI 1.28-2.83), 0.001] after adjusting confounders. model indicated a linear increase risks in-hospital higher index. revealed consistent effect sizes directions subgroups, ensuring result stability. results our suggest significant finding implies could potentially serve as valuable tool identifying AF at elevated

Cardiogenic shock (CS) is a severe cardiac disorder with high mortality rate. The triglyceride-glucose (TyG) index, biomarker of insulin resistance, associated cardiovascular disease-related mortality. This study aimed to investigate the association between TyG index and in patients CS. retrospective cohort analyzed 727 CS from Medical Information Mart for Intensive Care IV database. was calculated as follows: ln[triglycerides (mg/dL) × fasting blood glucose (mg/dL)/2]. Outcomes included 28-day intensive care unit (ICU) in-hospital Kaplan-Meier survival curve models Cox proportional hazards regression were used evaluate prognostic significance index. Receiver Operating Characteristic (ROC) analysis determine predictive efficacy Subgroup analyses conducted across different groups. Non-survivors had significantly higher (ICU: 9.30 vs. 9.13, p = 0.008; in-hospital: 9.29 0.004). Adjusted showed that each 1-unit increase increased ICU risk by 24% (hazard ratio [HR] 1.24, 95% confidence interval [CI]:1.04-1.48; 0.015) 44% (HR 1.44, CI:1.11-1.88; 0.007). Quartile 4 77% 1.77, CI:1.09-2.89) compared 1 61% 1.61, CI:1.08-2.38). area under ROC (AUROC) modest standalone ability 0.56, but when combined clinical variables, AUROC improved 0.80 0.78 (in-hospital). identified stronger associations ≥ 60 years, females, non-septic, those acute myocardial infarction or heart failure. short-term may serve useful stratification. Background: There is lack of predictive models for the risk severe complications during hospitalization in patients with acute myocardial infarction (AMI).In this study, we aimed to create a nomogram forecast likelihood in-hospital AMI.Methods: From August 2020 January 2023, 1024 AMI including modeling group (n=717) and validation (n=307) admitted Changsha Central Hospital's emergency department.Conduct logistic regression analysis, both univariate multivariate, on pertinent patient data from cohort at admission, identify independent factors, AMI, assess accuracy graph's predictions cohort. Results: Age, heart rate, mean arterial pressure, diabetes, hypertension, triglycerides white blood cells were seven factors serious patients.Based these variables, model was constructed. The has high (AUC=0.793for AUC=0.732 group). The calibration curve demonstrates strong consistency between anticipated observed values cohorts. Moreover, DCA results show that wide threshold range (0.01-0.73) good practicality clinical practice. Conclusion:This study developed validated an intuitive assist clinicians evaluating probability using readily available laboratory parameters.
