As the population ages, occurrence of osteoporosis is becoming more common. Diabetes mellitus one factors in development osteoporosis. Compared with general population, incidence significantly higher diabetic patients. Diabetic (DOP) a metabolic bone disease characterized by abnormal tissue structure due to hyperglycemia and insulin resistance, reduced strength increased risk fractures. This complex mechanism that occurs at cellular level such as blood vessels, inflammation, resistance. Although application some drugs clinical practice can reduce DOP, fractures caused DOP still very high. Extracellular vesicles (EVs) are new communication mode between cells, which transfer miRNAs proteins from mother cells target through membrane fusion, thereby regulating function cells. In recent years, role EVs pathogenesis has been widely demonstrated. this article, we first describe changes microenvironment Second, DOP. Finally, summarize research progress challenges

Osteoporosis (OP) is a systemic disease characterized by decreased bone density and quality, leading to fragile bones osteoporotic fractures (OPF). Conventional treatments for OPF often exhibit limited therapeutic efficacy significant side effects. Synthetic biology-based bacterial extracellular vesicles (BEVs) offer safe effective alternative treatment. Here, we constructed bioengineered BEVs loaded with pBMP-2-VEGF (BEVs-BP) encapsulated them together IL-4 in GelMA hydrogels form IL-4/BEVs-BP@GelMA. Initially, alleviated chronic inflammation modulating immune cells, while BEVs-BP subsequently enhanced osteogenesis vascularization upregulating BMP-2 VEGF expression. In vitro, IL-4/BEVs-BP@GelMA polarized M1 macrophages toward the M2 phenotype, osteogenesis, increased angiogenesis. Moreover, effectively promoted maturation mineralization of organoids vivo. Finally, successfully accelerated fracture repair mice. summary, developed an easy-to-build powerful biomaterial, IL-4/BEVs-BP@GelMA, which offers strategy management.

Rheumatic immune disorders are a group of conditions that affect the system, leading to various clinical symptoms. These diseases can cause pain, reduce quality life, and increase risk death in severe cases. Diagnosis treatment very complex due different types disease individual differences unknown pathogenesis disease. Further research is necessary provide new clues for treatment. Organoid technology makes up shortcomings animal model species better simulate onset mechanisms than models. It be used as screening platform therapeutic targets, well personalized settings based on patient-derived organoids, promising an effective tool study rheumatic diseases. Therefore, article summarizes studies related organoids their application also provides outlook potential this field discusses challenges need addressed, putting ideas future these
