Abstract Periodontitis is a prevalent and progressive detrimental disease which characterized by chronic inflammation, the immunopathological mechanisms being not yet fully understood. Mesenchymal stem cells (MSCs) play crucial roles as immunoregulators maintain tissue homeostasis regeneration, but their in vivo function in immunopathology periodontal deterioration still unclear. Here, we utilized multiple transgenic mouse models to specifically mark, ablate modulate Gli1+ cells, critical representative subset of MSCs periodontium, explore specific role immunopathology. We reveal that upon challenging an inflammatory microenvironment, significantly induce rapid trafficking aberrant activation neutrophils thus exacerbating alveolar bone resorption. Mechanistically, extracellular vesicles (EVs) released act immune regulators tissue, mediating recruitment through increased generation reactive oxygen species trigger nuclear factor kappa-B signaling. Importantly, inhibition EV release from or pharmacological therapy using GANT61 ameliorates inflammation loss. Collectively, our findings identify previously unrecognized orchestrating infiltration priming under provide pathological insights potential therapeutic targets for periodontitis.
