This article describes findings from the evaluation of Healthy Families NZ (HFNZ), an equity-driven, place-based community health initiative. Implemented in nine diverse communities across New Zealand, HFNZ aims to strengthen systems that can improve and well-being. Findings highlight local needs priorities including social mechanisms important for reorienting policy towards communities. Lessons encompass importance lived experience putting evidence into practice; strength acting with mind; need relational, learning, intentional, well-resourced organisation; examples how foster 'community-up' leadership; enable responsiveness between national systems. A reconceptualisation scaling context complexity change is offered, which recognises relationships agency are key making progress on determinants health.
