The C-reactive protein-albumin-lymphocyte (CALLY) index, a novel inflammation-immune-nutritional biomarker, has not been comprehensively evaluated for mortality risk prediction in older populations. Here, we investigate the relationship between CALLY index and all-cause Chinese adults aged ≥ 60 years. Data were obtained from 2014 to 2018 wave of Longitudinal Healthy Longevity Survey (CLHLS). Upon applying natural logarithmic transformation lnCALLY was stratified into tertiles. Kaplan-Meier analysis log-rank test employed assess cumulative survival probability across lnCALLY-stratified Cox proportional hazards regression utilized association mortality. Receiver operating characteristic (ROC) curves area under curve (AUC) values conducted evaluate predictive capacity Restricted cubic splines (RCS) with four knots applied explore potential non-linear dose-response Subgroup analyses sensitivity ensure validity. A total 1,738 participants included this cohort. Over median follow-up 3.3 years, 580 deaths (33.3%) occurred. multivariable demonstrated that highest tertile associated 40% reduced compared lowest [adjusted hazard ratio (HR) = 0.60, 95% confidence interval (CI): 0.49-0.73]. revealed significantly higher probabilities individuals elevated (P < 0.001). Time-dependent ROC showed AUC predicting at 1-, 2-, 3-year 0.751, 0.746, 0.762, respectively. RCS an approximate "L"-shaped negative correlation overall 0.001, P non- linearity 0.364). confirmed robustness, no significant interactions observed demographic or clinical strata. These findings suggest serves as practical prognostic biomarker monitoring populations, underscoring interplay inflammation, immunity, nutrition aging-related
