Abstract This study aimed to investigate the diverse clinical manifestations and simple early biomarkers predicting mortality of COVID-19 patients admitted emergency department (ED). A total 710 with were enrolled from 6,896 presenting ED between January 2022 March 2022. During period, a 478 tested positive for COVID-19, among whom 222 (46.4%) presented extrapulmonary COVID-19; 49 (10.3%) displayed gastrointestinal manifestations, followed by neurological (n = 41; 8.6%) cardiac 31; 6.5%). In total, 54 (11.3%) died. Cox proportional hazards model revealed that old age, acute kidney injury at presentation, increased leukocyte counts, low platelet decreased albumin levels, LDH levels independent predictors mortality. The exhibited highest area under curve in receiver operating characteristic analysis, value 0.860 (95% confidence interval, 0.796–0.875). showed presentations simple-to-measure prognostic markers ED. Serum can serve as novel biomarker identify high risk death.
