Abstract Effective leadership is essential in neonatal intensive care units (NICUs), where complex, high-stakes environments require coordinated multidisciplinary teamwork. Strong improves clinical outcomes, team performance, and staff well-being. This systematic review assesses various models interventions NICUs to identify best practices areas for future research. A search was conducted on PubMed, Web of Science, Scopus, covering studies published from 2010 October 2024. Articles were screened using the PRISMA guidelines, inclusion criteria focused primary NICU settings evaluating interventions. Data extraction quality assessment performed Newcastle–Ottawa Scale. Nine diverse countries research designs included. Leadership varied simulation-based training programs co-leadership models. High-fidelity simulation boot camps significantly improved self-perceived skills, teamwork, confidence among trainees. While most reported positive impacts performance patient safety, one large-scale improvement program showed no significant outcomes very-low-birth-weight infants.
