Importance Despite growing national concern about high and increasing rates of pregnancy-associated mortality due to homicide, suicide, drug overdose, state-level incidence has previously not been available. Objective To identify cases deaths involving firearms in the US from calendar year 2018 2022 estimate 5-year proportionate ratios per 100 000 live births by state cause death. Design, Setting, Participants This cross-sectional study is a population-based analysis 2018-2022 restricted-use files provided National Center for Health Statistics. These data include all occurring US, with geographic identifiers residence. All records which decedent was female aged 10 44 years pregnant at time death or up 1 earlier were included analysis. Data analyzed July December 1, 2024. Main Outcomes Measures International Statistical Classification Diseases Related Problems, Tenth Revision codes underlying used each 2022. Proportionate estimated as count divided total postpartum women state. Cause-specific Results Nationally, there 715 people who within post partum 2022, including 837 homicides, 579 suicides, 2083 overdoses, 851 that involved firearms. overdose varied across US. Of states more than 9 cases, homicide highest Mississippi (12.86 births), suicide Montana (21.55 Delaware (36.03 births). Firearms accounted many 15.56% Colorado, firearm (13.42 Conclusions Relevance The information this may provide relevant guidance local intervention strategies advance health, safety, well-being during pregnancy beyond.
