Reproduction consists of sequential inflammation-like events, primarily within the endometrium, from ovulation to embryo implantation, decidualization and delivery. During reproductive cycle, endometrium repeatedly undergoes cyclic periods proliferation, differentiation, tissue breakdown repair without scarring. Owing their phagocytic activity, macrophages, key players in innate immunity, are thought play crucial roles endometrium. Endometrial macrophages actively participate various stages remodeling, particularly during pregnancy establishment. Traditionally considered simple bystanders that clear debris prevent autoimmune responses homeostasis, now recognized as main actors with broad functional plasticity allows them fine tune balance between pro- anti-inflammatory inflammation, remodeling repair. Homeostatic is determined by sum mediators produced two distinctly polarized macrophage subpopulations. The biased polarization tissue-resident may contribute pathogenesis diseases, such inflammation cancer. Thus, understanding how endometrial homeostasis for deciphering underlying mechanisms disorders. Nanomedicines using extracellular vesicles, nanoparticles noncoding RNAs have recently been applied modulate alleviate disease phenotypes. Despite these advances, functions under physiological pathophysiological conditions remain poorly understood, which complicates development targeted therapies. Here we update current homeostatic function putative contribution dysfunction disorders women, along innovative molecular therapeutics resolve this issue.

Precisely timed immune adaptations, observed in the maternal circulation, underpin notion of an clock human pregnancy that supports its successful progression and completion at delivery. This is divided into three immunological phases, with first phase starting time conception implantation, shifting second homeostasis tolerance throughout pregnancy, culminating last labor parturition. Disruptions this are reported complications such as spontaneous preterm birth. However, our understanding preceding birth remains scattered. In review, we describe chronology cell adaptations during healthy pregnancies highlight disruption With a focus on single-cell cytometric, proteomic transcriptomic approaches, review recent studies term discuss need for future prospective aimed tracking longitudinally multi-omic scale. Postpartum hemorrhage (PPH) is the leading cause of maternal mortality worldwide. However, mechanism underlying atonic PPH remains partially elucidated. Multi-omics revealed that differentially expressed proteins and metabolites were enriched in immune-inflammation pathway vaginal blood patients with PPH. There was a pro-inflammatory immune microenvironment primarily activated by M1 macrophages decidua PPH, which presented as increased tumor necrosis factor (TNF)-α, interleukin (IL)-6, IL-8 levels affected contraction uterine smooth muscle. Besides, decidual macrophage group exhibited oxidative stress. The cell culture medium induced polarization peripheral monocytes towards while markedly increasing reactive oxygen species superoxide anion radical. Using hydrogen peroxide (H2O2) to stimulate similar state samples, secretion cytokines, such TNF-α IL-8, significantly upregulated, impacted expression contraction-associated (CAPs) muscle cells (uSMCs). animal model suggested H2O2 promoted placental M1, affecting stress inflammatory infiltration, contractility tissues. In summary, abnormal at maternal-fetal interface macrophages, causing cytokines. acted on uSMCs inhibit CAP expression, inducing

Most pharmacotherapeutic chemicals/interventions used to manage preterm labor (PTL) often cause neonatal morbidity and maternal adverse reactions. Fructooligosaccharides, extracted from traditional Chinese medicine, can alleviate inflammation, demonstrate antiviral capabilities, protect against antioxidant stress, implying a potential effective PTL treatment. In this study, we explored the protective effects of purified burdock fructooligosaccharide (BFO), Gfn-type fructose polymer, on inflammation-induced PTL. It was found that two doses 30 mg/kg mouse BFO administration pregnant mice at 6 h interval effectively ameliorate lipopolysaccharide (LPS)-induced Drug dynamic distribution analysis revealed rather highly enriched in myometrial tissues, could inhibit oxytocin-induced uterine smooth muscle contraction, bind toll-like receptor 4 (TLR4) membrane cells, downregulating expression downstream genes, attenuating upregulation inflammatory cytokines serum myometrium, as well reversing increased macrophage neutrophil infiltration into myometrium induced by LPS. also interfere with levels estrogen progesterone, alleviating occurrence premature birth. These findings collectively suggest might serve promising therapeutic agent for inflammation-related safeguard health both mother fetus.
