Background: Immunization is a cornerstone of public health, aiming to reduce morbidity and mortality among children. Despite global efforts increase vaccination coverage, gaps in immunization persist, influenced by variety socio-economic demographic factors. Understanding these factors crucial for improving strategies achieving herd immunity. Objective: The study aimed assess the coverage children aged 12 23 months identify associated with incomplete peri-urban area Peshawar. Methods: A cross-sectional survey was conducted from September 15, 2021, November 2022, involving 239 five villages selected through multi-stage cluster sampling. Data on factors, along status, were collected interviews mothers caretakers using semi-structured questionnaire. assessed, failure complete analyzed. Statistical analysis performed SPSS version 25. Results: revealed that 50.2% (120/239) fully immunized, 35.5% (85/239) partially 14.2% (34/239) unimmunized. Factors contributing included lack information (31/239), motivation (17/239), accessibility issues (9/239), vaccine availability (7/239), family-related challenges (22/239). No significant gender discrimination observed. Children families lower socioeconomic status parents education less than secondary level more likely be incompletely immunized. Conclusion: While there has been progress barriers remain, particularly low backgrounds levels parental education. Addressing targeted health interventions enhanced community awareness essential protecting against vaccine-preventable diseases.
