The aim of this report is to present a successful combined surgical and pharmaceutical treatment in the highest stage medicine-related osteonecrosis jaw (MRONJ). A 70-year-old man treated for metastatic prostate cancer concomitant with hypertension diabetes presented due exposure jawbone. Initial imaging studies suggested MRONJ, biopsy did not confirm bone metastasis oral cavity. Marginal resection mandible was performed after administration antibiotics anticoagulants. There no recurrence mandibular necrosis during 3-year follow-up. MRONJ can develop covertly, scanty clinical symptoms, be easily overlooked. Radical may, some cases, prevent further progression disease, which case.
