Background: Post-extraction ridge resorption is an inevitable phenomenon that cannot be eliminated but significantly reduced using additional surgical techniques known as socket preservation. They aim to create favorable conditions for implant placement and prosthetic restoration. This study aims assess the effect of sealing (SS) with free gingival grafts on vertical walls at premolar molar regions over 3 months. Methods: randomized two-arm controlled trial parallel groups (1:1 allocation) was conducted Department Oral Surgery, Medical University-Varna, Bulgaria, from 27 June 2022 20 April 2023. Forty patients aged 30-65 were equally randomly allocated SS or control groups. Atraumatic tooth extraction performed. In dental implantology, alveolar ridge preservation (ARP) has emerged as a standard technique to address dimensional changes that affect morphology following tooth loss. Various alternative graft materials, including xenografts, alloplasts, and allografts, have been effectively employed in fresh extraction sites for ARP. Current evidence suggests these materials primarily serve bio-scaffolds, which are slowly incorporated, thus necessitating waiting period of at least 4–6 months before implant placement. Consequently, the ARP extends overall duration treatment by several months. Recently, incorporation form autologous platelet concentrate, known platelet-rich fibrin (PRF), advocated conjunction with method bioenhancement soft- hard-tissue healing regeneration. The objective of the study was to evaluate long-term esthetic and radiographic results implants placed in anterior maxilla after ridge preservation, combining bovine xenograft with collagen matrix. Fifteen patients who required a single tooth extraction because fracture, root resorption, or extended caries were included study. After extraction, all sites grafted using Deproteinized Bovine Bone Mineral (DBBM) covered by resorbable matrix (CM). Five months socket grafting, successfully installed. implant diameter range between 3.8 4.2 mm. All monitored for over 7 years, both clinically radiographically. Three independent observers evaluated outcome, employing Pink Esthetic Score (PES) technique. Over period exceeding seven 100% survival rate observed 15 implants, minimal marginal bone loss. mean PES 11.40 (±1.44) at first assessment 11.38 (±1.63) second assessment. difference not statistically significant (p = 0.978), scores measurements indicated excellent even years. Based on these preliminary results, it seems that placing fresh socket, matrix, can preserve alveolar provide stable results.
