The cold agglutinin syndrome is a haemolytic disorder usually manifested by acrocyanosis and Raynaud's phenomenon. Gangrene an uncommon complication, associated with infections or B-cell lymphoproliferative diseases. We present case of fulminant gangrene fingers, toes, nose in 77-year-old woman atypical pneumonia acute renal failure. diagnosis anaemia was done the treatment antibiotics corticosteroids effective. However, amputation all toes on both feet could not be avoided. Clinical aspects agglutinemia, transient chronic, are briefly discussed. conclude that, presence syndrome, should promptly initiated order to avoid complications such as extensive gangrene.

Autoimmune hemolytic anemias (AIHAs) are conditions whereby a person produces an antibody to their own red blood cells (RBCs). These can be primary with unknown cause or secondary, associated diseases infections. There seven different categories of AIHA, warm-antibody AIHA (wAIHA); cold-antibody that includes cold agglutinin syndrome (CAS) and agglutin disease (CAD); mixed also called combined warm AIHA; paroxysmal hemoglobinuria (PCH) termed Donath-Lansteiner test-positive direct antiglobulin test-negative (DAT-neg AIHA); drug-induced immune anemia (DIIHA); passenger lymphocyte syndrome. This review will discuss each these AIHAs provide information on diagnosis treatment.
