The world of science is increasingly looking for answers to the question how specific dietary components affect risk chronic diseases and assessing their potential alleviate severity individual diseases. This research concerns both assessment benefits using those nutritional models. Background Oxidative stress plays a crucial role in the female reproductive system. Background Composite dietary antioxidant index (CDAI) has been found protective to women’s health. However, the association between CDAI level and risk of endometriosis in women is unclear. Methods A total 4,153 from National Health Nutrition Examination Survey (NHANES) 2001–2006 were included this cross-sectional study. We evaluated using three logistic regression models restricted cubic spline. Stratified sensitivity analyses also performed. Results Logistic analysis that was inversely associated with development endometriosis. The odds ratio (OR) for each SD increase 0.98 [95%CI: 0.96–0.99]. After dividing into four quartiles, we compared Q1 (−1.89, −1.79), ORs [95%CI] Q2 (−1.79, −0.69), Q3 (−0.69, 1.42) Q4 (1.42, 47.92) 0.94 [0.87, 1.03], 0.95 [0.88, 1.04] 0.83 [0.77, 0.91], respectively, p trend<0.001. Infertility affects one in six individuals globally, with oxidative stress-modifiable through diet and lifestyle-being a key factor female infertility. This study examines associations between dietary/lifestyle Oxidative Balance Scores (OBS) infertility risk reproductive-aged women. Using 2013-2018 NHANES data from 2,447 women aged 20-44, we calculated OBS (16 dietary 4 lifestyle components) reflecting antioxidant vs. pro-oxidant exposure. Higher scores indicate dominance. Weighted logistic regression models adjusted for sociodemographic health covariates revealed an inverse linear relationship: each 1-unit increase correlated 4.7% lower (95% CI: 0.929-0.977, p < 0.001). Women the highest tertile had 6.4% reduced odds versus tertiles 0.897-0.977, = 0.003). Joint analyses showed higher mitigated among low OBS. Results remained consistent across subgroups sensitivity analyses. These findings underscore potential of antioxidant-rich diets healthy lifestyles reducing stress-related infertility, advocating targeted interventions to improve reproductive outcomes.
