Despite the increasing recognition of interplay between depression and cardiovascular disease (CVD), precise mechanisms by which contributes to pathogenesis remain inadequately understood. Abstract Platelet hyperreactivity contributes significantly to thrombosis in acute myocardial infarction and stroke. While antiplatelet drugs are used, residual ischemic risk remains. Intermittent fasting (IF), a dietary pattern characterized by alternating periods of eating fasting, has shown cardiovascular benefits, but its effect on platelet activation is unclear. This study demonstrates that IF inhibits both patients with coronary artery disease apolipoprotein E (ApoE) knockout (ApoE−/−) mice, enhancing intestinal flora production indole-3-propionic acid (IPA). Mechanistically, elevated IPA plasma directly attenuates binding the pregnane X receptor (PXR) suppressing downstream signaling pathways, including Src/Lyn/Syk LAT/PLCγ/PKC/Ca2+. Importantly, alleviates cerebral ischemia/reperfusion injury ApoE−/− mice. These findings suggest mitigates atherosclerosis IPA, which subsequently activates PXR-related pathways.

Diabetic kidney disease (DKD) is one of the most severe complications diabetes mellitus, with limited effective therapeutic interventions. Alternate-day fasting (ADF) shows potential in treating DKD, though its mechanisms are not fully understood. In this study, BTBR ob/ob mice underwent 12 weeks ADF, and high-resolution untargeted metabolomics were performed to uncover underlying mechanisms. After exhibited weight loss, lower blood glucose LDL-C levels, reduced 24-h urinary protein excretion, decreased renal collagen deposition. A total 44 metabolites differentially expressed, 25 up-regulated 19 down-regulated. Notably, hyocholic acid (HCA) indole-3-propionic (IPA), both products intestinal bacteria, can modulating inflammation expressed. Furthermore, kidneys showed significantly NF-κB pathway activity after ADF. This study indicates that ADF may alleviate DKD progression by HCA, IPA, decreasing inflammation.
