ABSTRACT Objectives The increase in polypharmacy among older adults increases the risk of drug‐related problems, making multidisciplinary interventions essential. This study evaluated impact a consultation on medication management and outcomes outpatients. Methods prospective observational at Spanish teaching hospital involved geriatricians, clinical pharmacists, nurses. Older (≥ 75 years) with underwent review baseline 3 6 months.
