Foreign body aspiration in pediatric patients presents a significant challenge diagnosis and management, often leading to acute respiratory distress. We report the case of 4-and-a-half-year-old male child with history recurrent left-sided lobar pneumonia, who presented high-grade fever cough persisting for 14 days. Despite lacking distress or stridor, routine chest X-ray revealed right-sided lower lobe prompting further investigation. Auscultation reduced air entry crepitations right lobe. Bronchoscopy confirmed presence deep-seated foreign (a wheel toy car its axle) main bronchus. Prompt intervention by an ENT surgeon led successful removal under general anesthesia. Post-procedural care included intravenous administration dexamethasone ceftriaxone, along budecort nebulization. This highlights complexities diagnosing emphasizes importance prompt prevent complications ensure favorable outcomes.
