Abstract The emergence of large language models (LLM) with remarkable performance such as ChatGPT and GPT-4, has led to an unprecedented uptake in the population. One their most promising studied applications concerns education due ability understand generate human-like text, creating a multitude opportunities for enhancing educational practices outcomes. objective this study is twofold: assess accuracy ChatGPT/GPT-4 answering rheumatology questions from access exam specialized medical training Spain (MIR), evaluate reasoning followed by these LLM answer those questions. A dataset, RheumaMIR, 145 rheumatology-related questions, extracted exams held between 2010 2023, was created that purpose, used prompt LLM, publicly distributed. Six rheumatologists clinical teaching experience evaluated chatbots using 5-point Likert scale degree agreement analyzed. association variables could influence models’ (i.e., year question, disease addressed, type question genre) studied. demonstrated high level both accuracy, 66.43%, reasoning, median (Q1–Q3), 4.5 (2.33–4.67). However, GPT-4 showed better score 93.71% value 4.67 (4.5–4.83). These findings suggest may serve valuable tools education, aiding preparation supplementing traditional methods.
