Despite being a leading cause of chronic pain and disability, the underlying mechanisms intervertebral disc (IVD) degeneration (IVDD) remain unclear. Emerging evidence suggests that mechanosensation (the ability skeletal system to perceive mechanical biochemical signals) mediated by abnormal loading plays critical role in regulation IVD health. This review examines complex interactions amongIVDs, intraosseous sensory mechanisms, central nervous (CNS), with particular focus on roles pathways such as PGE2/EP4, Wnt/β-catenin, NF-κB. elucidates manner which stress aberrant signaling disrupt homeostasis nucleus pulposus (NP), cartilaginous endplate (CEP) annulus fibrosus (AF), thereby driving exacerbating pain. Furthermore, targeted therapeutic strategies, including modulation interoception dynamic loading, present novel avenues for reversing IVDD progression. By integrating biology spinal pathology, this work offers perspective pathogenesis identifies promising strategies clinical intervention. These findings highlight potential targeting mitigate associated pain, paving way innovative, mechanism-driven therapies.

Low back pain (LBP) is very common in women of child bearing age (WCBA) and a major burden on individuals society, but studies about it have not been reported. Trend analyses were based data from the Global Burden Disease, Injuries, Risk Factors Study (GBD) 2021. We investigated global trends number prevalence cases, years lived with disability (YLDs), YLDs rate for LBP WCBA. used joint point regression to report average annual changes identify most variable trend. Simultaneously, we further analyzed these by stratifying them region, nation, age. Globally, increased substantially 1990 2021(the prevalence: 39.65%) (the YLDs: 39.53%), whereas WCBA decreased AAPC: -0.1355 [95% CI -0.1419 -0.1288) -0.1365 -0.1427 -0.1302]). Regionally, among medium socio-demographic index region showed an increasing trend(the 0.1291 CI: 0.118 0.1396]) 0.1371 0.1251 0.1499]). Nationally, Thailand Vietnam countries largest increases YLD rates(Thailand: 0.7742 0.7589 0.7904]) (Vietnam: 0.7364 0.7263 0.7468]). In patterns, highest rates found 45–49 group(the 14,412.77 per 100,000 2021 UI: 10,482.27 19,077.3]) 1,639.56 population (95% 1,035.24 2,494.9]). Despite declines LBP, remains high. Age- region-specific prevention healthcare strategies should be optimized meet needs reduce disease.
