Abstract Background Despite the significance of health literacy, studies show inadequacy literacy in system Iran. Inadequate is considered a warning for service providers. It needs to be integrated educational programs run and medical centers. Therefore, present study was conducted explore barriers Iran system. Methods The qualitative 2023 using conventional content analysis. To this aim, 18 subjects were selected among urban center staff, 21 from rural centers, 10 volunteers 8 trainees interviews group discussions. data collected through semi-structured focal discussions, simultaneously analyzed analysis suggested by Lundman Graneheim. Results led extraction 4 themes: personal (trainee’s inadequate competence, lack physical or mental health, low motivational factors, wrong beliefs, contextual barriers), interpersonal (healthcare providers’ misunderstanding correct training, mal- miscommunication training sessions, high workload healthcare trainer’s limited skill, underestimated role education behavior change, supervision monitoring problems, work commitment staff), organizational (inadequate human resources, poor management, motivation) social (uninterest learning about issues, issues related Internet cyberspace, intersectoral cooperation, distrust system). Conclusion Considering multidimensional nature it recommended make implement comprehensive plan integrate efforts authorities institutions, mass media, experts, decision-makers, policy-makers, providers remove barriers.
