Background: Rheumatoid arthritis (RA) is a chronic autoimmune disease that affects 1% of the world's population. It causes inflammation synovial tissue, leading to joint destruction, poor quality life, and disability. Cathepsin G (CTSG), which acts as proteolytic enzyme, can be factor in RA. These proteases belong serine protease family have role disorders. They cause bone cartilage destruction an immune response cases inflammatory arthritis. The objective this study determine if CTSG could serve potential biomarker for diagnosis evaluation activity Methods: involved 132 patients with arthritis, their serum levels anti-citrullinated peptide antibody (ACPA) were measured using ELISA. Other routine biomarkers, including C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), rheumatoid (RF), also measured. Results: mean was significantly lower RA (110.53 pg/ml ± 49.959) than those other types (132.65 30.199). According DAS-28 ESR CRP, found no significant difference across four groups (P = 0.585, P 0.823, respectively). Additionally, had negative correlation diabetes mellitus treatment intake newly diagnosed 0.009, 0.041, This first evaluate diagnostic tool, showing sensitivity 70.1% specificity 60.0% at cut-off value ≤133.33 pg/ml.
