Worldwide, sepsis is a well-recognized cause of death. Acute kidney injury (AKI) may be related to in up 70% AKI cases. Sepsis-associated (SA-AKI) defined as the presence according Kidney Disease: Improving Global Outcomes criteria context sepsis. SA-AKI categorized into early, which presents during first 48 h sepsis, and late, presenting between 7 days associated with worse prognosis among patients However, there are different phenotypes well pathophysiological pathways SA-AKI. The aim this review provide an updated synopsis pathogenetic mechanisms underlying development analyze its prognosis. In addition, potential novel diagnostic prognostic biomarkers therapeutic approaches discussed. A plethora implicated pathogenesis SA-AKI, including inflammation metabolic reprogramming sepsis; various types cell death such apoptosis, necroptosis, pyroptosis ferroptosis; autophagy efferocytosis; hemodynamic changes (macrovascular microvascular dysfunction). Apart from urine output serum creatinine levels, have been incorporated definition AKI, several urinary also developed, comprising, others, interleukins 6, 8 18, osteoprotegerin, galectin-3, presepsin, cystatin C, NGAL, proenkephalin A, CCL-14, TIMP-2 L-FABP stemming multi-omics technologies machine learning algorithms. Interestingly, long non-coding RNAs (lncRNAs) microRNAs (miRNAs), PlncRNA-1, miR-22-3p, miR-526b, LncRNA NKILA, miR-140-5p miR-214, serve targets. combination omics represents innovative holistic approach toward providing more integrated view molecular physiological events for deciphering unique specific phenotypes. Although evidence still necessary, it expected that incorporation integrative useful not only early diagnosis risk but targets could revolutionize management personalized manner.

Sirtuins (SIRTs) represent a class of nicotinamide adenine dinucleotide (NAD+)-dependent protein deacetylases that exert crucial role in cellular signal transduction and various biological processes. The mammalian sirtuins family encompasses SIRT1 to SIRT7, exhibiting therapeutic potential counteracting aging, modulating metabolism, responding oxidative stress, inhibiting tumors, improving microenvironment. These enzymes are intricately linked the occurrence treatment diverse pathological conditions, including cancer, autoimmune diseases, cardiovascular disorders. Given significance histone modification gene expression chromatin structure, maintaining equilibrium is imperative for disease prevention health restoration. Mounting evidence suggests modulators SIRTs play treating diseases physiological balance. This review delves into molecular structure regulatory functions family, reviews classification historical evolution modulators, offers systematic overview existing modulation strategies, elucidates mechanisms (agonists inhibitors) their clinical applications. article concludes by summarizing challenges encountered modulator research offering insights future directions.

Abstract In sepsis, inflammation, and nutrient deficiencies endanger cellular homeostasis survival. Autophagy is primarily a mechanism of survival under fasting conditions. However, autophagy-dependent cell death, known as autophagic proinflammatory can exacerbate sepsis. also regulates various types non-inflammatory inflammatory deaths. Non-inflammatory apoptosis tends to suppress however, necroptosis, pyroptosis, ferroptosis, death lead the release cytokines damage-associated molecular patterns (DAMPs) amplify inflammation. The selection mechanisms complex often involves mixture styles. Similarly, protective autophagy lethal may be triggered simultaneously in cells. How cells balance regulatory these processes an area interest that still investigation. Therapies aimed at modulating are considered promising. Enhancing helps clear recycle damaged organelles reduce burden while inhibiting excessive autophagy, which could prevent death. this review, we introduce recent advances research system

Tiliroside, a natural flavonoid, has various biological activities and improves several inflammatory diseases in rodents. However, the effect of Tiliroside on lipopolysaccharide (LPS)-induced acute kidney injury (AKI) underlying mechanisms are still unclear. This study aimed to evaluate potential renoprotective LPS-induced AKI mice. Male C57BL/6 mice were intraperitoneally injected with LPS (a single dose, 3 mg/kg) or without (50 200 mg/kg/day for 8 days). administration protected against AKI, as reflected by ameliorated renal dysfunction histological alterations. LPS-stimulated expression cytokines, fibrosis markers, markers was significantly abolished Tiliroside. flavonoid also stimulated autophagy flux but inhibited oxidative stress tubular cell apoptosis kidneys from LPS-injected Mechanistically, our showed regulation intrarenal renin-angiotensin system treatment suppressed AGT, Renin, ACE, Ang II, upregulated ACE2 Ang1-7, affecting plasma II Ang1-7 levels. Collectively, data highlight action suppressing inflammation, stress, activating via shift towards ACE2/Ang1-7 axis away ACE/Ang axis.

Purpose: To address the critical mortality rates among sepsis-associated acute kidney injury (SA-AKI) patients, early prognosis is vital. This study investigates relationship between coagulation indices and 28-day rate in patients with SA-AKI. Patients Methods: was a retrospective cohort analysis including SA-AKI admitted to First Hospital of Fujian Medical University as training (n = 119) Third People's Traditional Chinese Medicine validation 51). We examined SA-AKI, cumulative at different activated partial thromboplastin time (APTT) levels, nonlinear APTT mortality. Receiver operating characteristic curves were plotted, area under curve calculated assess predictive power APTT. Finally, subgroup analyses performed robustness association. Results: Overall, 119 participants mean±standard deviation age 70.47± 15.20 years included cohort: 54 died, 65 survived. According univariate multivariate COX regression analyses, APACHE II score, CRP level, Lac level independent risk factors for adverse prognosis. After controlling some variables, an elevated baseline (≥ 37.7 s) associated (HR, 1.017; 95% CI, 1.001– 1.032), Kaplan–Meier further confirmed increased group higher The same results shown when analyzed 1.024; 0.958– 1.096). Subgroup showed stability association poor Conclusion: In essence, elevation synonymous rates, indicating scenarios. Keywords: APTT, prognosis, studies, illness
