Cardiopulmonary bypass (CPB) triggers marked cytokine release often followed by a systemic inflammatory response syndrome, associated with adverse postoperative outcomes. This trial investigates the intraoperative use of haemoadsorption (HA) during cardiac surgery CPB to assess its impact on response. In this prospective randomised controlled (ethics approval no. 5094-14DRKS00007928), patients (> 65 years) undergoing elective on-pump were HA (CytoSorb) or standard care without HA. Primary outcome was difference in mean interleukin (IL)-6 serum concentrations between groups intensive unit (ICU) admission. The secondary outcomes included various clinical and biochemical endpoints. Statistical methods paired unpaired t-tests, Wilcoxon, Mann–Whitney U-tests, chi-square tests. Thirty-eight allocated receive either (n = 19) 19). primary outcome, IL-6 levels ICU admission, did not differ study group controls (214.4 ± 328.8 vs. 155.8 159.6 pg/ml, p 0.511). During pre- versus post-adsorber IL-2, IL-6, IL-8, IL-10, heparan sulfate myoglobin post- reduced. Furthermore, day 1 2 ICU. While sequential organ failure assessment scores, lactate levels, C-reactive protein procalcitonin (PCT) showed no statistically significant differences. Regarding haemodynamic stability treatment index (3.2 0.7 2.47 0.47 l/min/m2, 0.012) increased, lower fluid requirements as well decreased fibrinogen requirement observed. Need for renal replacement therapy though shorter duration observed group. Abstract Tumor immunotherapy has significantly transformed the field of oncology over past decade. An optimal tumor would ideally elicit robust innate and adaptive immune responses within microenvironment (TIME). Unfortunately, system experiences functional decline with chronological age, a process termed “immunosenescence,” which contributes to impaired against pathogens, suboptimal vaccination outcomes, heightened vulnerability various diseases, including cancer. In this context, we will elucidate hallmarks molecular mechanisms underlying immunosenescence, detailing alterations in immunosenescence at molecular, cellular, organ, disease levels. The role tumorigenesis senescence‐related extracellular matrix (ECM) also been addressed. Recognizing that is dynamic influenced by factors, evaluate treatment strategies targeting mechanisms, as well methods for cell, organ restoration, present emerging approaches immunotherapy. overarching goal research prevent development, recurrence, metastasis, ultimately improving patient prognosis. Our review aims reveal latest advancements prospective directions research, offering theoretical basis development practical anti‐immunosenescence anti‐tumor strategies.

The term 'inflammaging' has been coined to describe the chronic state of inflammation derived from ongoing cycles tissue damage and subsequent immune responses. This inflammatory status contributes decline organs physiological functions, accelerates aging process, increases risk age-related illnesses death. During aging, gut microbiota (GM) undergoes significant changes, including a decreased diversity species, in beneficial bacteria, rise proinflammatory ones, resulting persistent low-grade inflammation. Moreover, environmental factors, such as diet medications, contribute changes GM function, preventing or promoting inflammaging. narrative review aims clarify underlying mechanisms inflammaging specifically investigate influence several factors on these mechanisms, while also exploring potential differences related sex. lifestyle pharmacological interventions will be suggested promote healthy aging.
