Bullous pemphigoid (BP) is a chronic autoimmune disorder characterized by subepidermal blister formation, primarily affecting elderly individuals. While BP has been associated with malignancies, the exact nature of this relationship remains unclear. We report case 72-year-old man who presented pruritic cutaneous lesions, including tense vesicles and bullae was diagnosed BP. Despite treatment doxycycline, nicotinamide, topical clobetasol, his symptoms persisted. A routine chest X-ray, conducted as part diagnostic workup, incidentally, revealed large mass in right upper lung, which subsequently squamous cell carcinoma. Remarkably, after surgical resection tumor, patient's lesions completely resolved within seven weeks, without alterations to dermatologic treatment. This emphasizes importance malignancy screening patients persistent suggests possible link between underlying cancer, particularly when standard therapies prove ineffective. Further investigation into mechanisms connecting these conditions warranted.
