Inspiratory muscle training (IMT) has been to be effective improving respiratory functions; however, no studies have compared the effects of incentive spirometer (IS) threshold trainer (TIMT) PD. The study aimed compare IS TIMT on maximum pressure (MIP), 6-min walk distance (6-MWD), forced vital capacity (FVC), volume 1 s (FEV1), peak flow rate (PEFR) stage 1–3 according Hoehn Yahr scale. 18 were randomly assigned two groups, i.e., group. group received IMT volume-based IS, TIMT. MIP, 6-MWD, FVC, FEV1, PEFR measured before after six weeks training. Abstract Hypokinetic dysarthria in Parkinson's Disease (HD-PD) deteriorates patients' quality of life by impeding communication and social engagement. Existing treatments like levodopa drugs deep brain stimulation (DBS) can improve motor symptoms but fall short addressing speech-related impairments; effective speech therapies tend not to mandate the posture, which potentially hinders vocal performance. Here we first proposed a therapeutic protocol that specified lunging-and-clawing posture (Wohu Pushi method from Yijinjing) during vocalization training. The aimed ensure PD patients could accomplish training even with moderate impairments. A total 20 HD-PD implanted stimulators subthalamic nuclei (STN-DBS) participated one session training, either following our posture-voice therapy (PVT) or conventional voice (CVT). Results indicated maximum phonation time was significantly elongated PVT(3.85±2.81s) CVT(0.46±2.24s), formant-distance increased PVT(95.80±112.71Hz) compare CVT(-47.10±84.28Hz). Our results demanding appropriate amount postural maintenance vocalization, it might facilitate recovery functions. This study warranted larger-scale clinical trials understand impact, limitation, optimization for HD-PD.

Background Up to 75% of patients with Parkinson disease (PD) experience voice and speech impairments, such as breathy phonation low volume, which worsen over time negatively impact the quality life. However, given their increasingly limited mobility, face-to-face therapy is often inaccessible. Mobile health (mHealth) apps offer accessible cost-effective alternatives; yet, application in PD-specific, self-delivered remains underexplored. Objective This study aimed evaluate feasibility, adherence, satisfaction a smartphone app for PD, designed minimize speech-language pathologist involvement while promoting patient independence. In addition, it seeks assess preliminary therapeutic effectiveness addressing problems this population. Methods A single-arm, rater-blinded, pretest posttest was conducted between September November 2023. Patients PD who have no problem using Android (Google) smartphones were recruited. Participants downloaded researcher-developed mHealth on participated patient-tailored 5-week home-based training program. Each session included 5 stages: breathing, oral motor exercises, loudness, prosody, functional speaking. The program consisted 20 sessions, participants completing 1 per day, 4 days week. lasted approximately 20-30 minutes. Adherence monitored through logs, assessed phone survey, evaluated acoustic analysis auditory-perceptual assessments. Results Out 30 initially recruited, but 2 them withdrew. 25 completed all sessions 3 dropped out. adherence above 90% (80%, 20/25), 70% (16%, 4/25), below 4/25). Satisfaction (18/24) among 24 people survey. Significant improvements observed measures: maximum increased from 11.15 (SD 5.38) seconds 14.01 5.64) (P=.003), vocal intensity 71.59 4.39) dB 73.81 3.48) (P<.001) across both sustained reading tasks. Voice scores GRBAS (grade, roughness, breathiness, asthenia, strain) scale improved significantly (all components P<.001). Furthermore, 58.3% (14/24) reported subjective voice. Conclusions demonstrates that home-based, self-training delivered feasible solution suggesting can serve convenient effective alternative by enhancing accessibility empowering actively manage condition.
