Patients with Parkinson’s disease (PD) face a multitude of gastrointestinal (GI) symptoms, including nausea, bloating, reduced bowel movements, and difficulties defecation. These symptoms are common may accumulate during the course PD but often under-recognized challenging to manage. Objective testing can be burdensome patients does not correlate well symptoms. Effective treatment options limited. Evidence is based on studies in general population, specific evidence scarce. Upper GI dysfunction also interfere pharmacological motor which poses significant management challenges. Several new less invasive assessment tools novel have emerged recent years. The current review provides an overview practical approach recognizing diagnosing upper lower problems PD, e.g., dyspepsia, gastroparesis, small dysfunction, chronic constipation, defecatory dysfunction. Management aspects discussed latest from populations, insights for future research pertaining PD.

Disordered airway protection, including both disordered swallowing (dysphagia) and cough (dystussia), is highly prevalent among persons with Parkinson's disease (pwPD). A comprehensive understanding of the perspectives pwPD as it relates to protection currently lacking necessary provide relevant, person-centered care. Therefore, this study used a parallel-convergent mixed methods design quantitatively qualitatively investigate pwPD's on protection. Thirteen were consecutively recruited from training trial. Quantitative data (obtained Swallowing Quality Life questionnaire [SWAL-QOL] modified Canadian Occupational Performance Measure [mCOPM]) qualitative semistructured interviews) integrated define participant knowledge, perceived importance, performance, psychosocial implications protective deficits. All participants reported basic knowledge about which centered around four themes-physiology, interconnectedness between bulbar functions, danger, impaired consequence PD. believed that was important (median mCOPM score = 10), three primary themes related importance-survival, anticipated future decline, maintaining oral intake. We identified split who themselves have "a problem" those did not, relating impairment-use eating strategies, unpredictable inconsistent symptom experience, vigilance required mitigate symptoms. Psychosocial ramifications key aligned SWAL-QOL domains-overall burden, fear social embarrassment. The integration quantitative in highlights importance considering unique patient develop personalized relevant management plans for each integrate objective clinical findings priorities needs. https://doi.org/10.23641/asha.28489280.

Background The complexity of gastrointestinal (GI) disorders associated with Parkinson's disease (PD) and the significant interactions between GI medications dopaminergic axis necessitates expert management. integrated care model for brain-gut interaction (DBGI) has advantages, however, not been applied in concurrent DBGI PD. Objective To test hypothesis that our Research Integrated Support Model (PRISM) will reduce symptom severity improve quality life (QOL) patients symptoms Methods Patients refractory referred to PRISM clinic were evaluated treated by efforts movement disorder specialists, neurogastroenterologists, dietitians, occupational therapists, speech-swallow neuroscientists. underwent a battery QOL questionnaires personalized actionable biomarkers (motility testing swallowing studies). Inflammatory markers stool tests collected. An individualized standard treatment was established based on specific diagnosis uncovered during evaluation. Results 44 adult PD complaints evaluated. most common included constipation (97%), dysphagia (61%), gastroesophageal reflux (34%). Actionable highly positive revealing esophageal dysmotility (20/21, 95%), slow-transit (40/42, 90%), intestinal methanogen overgrowth (7/8, 87%), gastroparesis (17/20, 85%), oropharyngeal (28/44, 63%), dyssynergic defecation (27/42, 61%). significantly improved ( p < 0.05) as measured all questionnaires. Conclusions More severely affected Fixel approach showed improvements frequency, severity, QOL.

Parkinson's disease (PD) is a complex neurodegenerative motor disorder caused by the loss of dopaminergic neurons in substantia nigra pars compacta. The neither first nor only brain region affected PD. Recent and old studies have shown that PD does not affect CNS; fact, autonomic innervation GIT, skin, olfactory system was found to be α-synuclein pathology outside CNS, affecting patients' quality life. In gastrointestinal system, dysphagia, constipation, bacterial overgrowth small intestine are common patients with addition, several skin conditions were reported PD, including seborrheic dermatitis, rosacea, melanoma, others. Finally, dysfunction, such as reduced touch sensation smell, associated abnormalities. Further high-quality needed develop reliable tests could help early diagnosis
