Alteplase is a widely used thrombolytic therapy for acute ischemic stroke (AIS). However, the efficacy of intravenous thrombolysis (IVT) with alteplase varies due to individual factors, and some patients are prone embolization recurrence (ER) after IVT. The mechanisms underlying these variations not fully understood. In this study, we analyzed changes in plasma lipid profiles AIS treated based on lipidomics, focusing association between lipids recurrence. Our findings revealed significant several species who experienced ER, including triglycerides (TG), diglycerides (DG), phosphatidylcholine (PC), sphingomyelin (SM). Notably, most TG DG levels ER group were significantly upregulated, PC SM downregulated. was upregulated 24h IVT treatment, while DG, SM, remained These differential metabolites mainly involved metabolism, immune response inflammation-related, insulin resistance signaling pathways. This study demonstrates that alterations, particularly TG, PC, may be associated treatment. further studies needed elucidate exact through which influence onset recovery AIS.
