Hemorrhagic transformation (HT) is a severe complication in patients with acute ischemic stroke (AIS) undergoing intravenous thrombolysis therapy (IVT). Epicardial adipose tissue (EAT) contributes to the development of AIS and disruption blood-brain barrier. Background/Objectives: Hemorrhagic transformation after endovascular thrombectomy predicts poor outcomes in acute ischemic stroke with large-vessel occlusion. The roles of microRNAs (miRNAs) the pathogenesis parenchymal hematoma (PH) still remain unclear. This study aimed to investigate miRNA and mRNA regulatory network associated PH mechanical reperfusion an animal model oxygen-glucose deprivation/reoxygenation (OGD/R) model. Methods: Twenty-five miRNAs were assessed a reperfusion-induced hemorrhage rats under hyperglycemic conditions receiving 5 h middle cerebral artery differentially expressed neuron, astrocyte, microglia, brain microvascular endothelial cell (BMEC), pericyte OGD/R. predicted target genes further miRNA-mRNA was established. Currently, the adjuvant therapy to optimize restorative process after stroke is required due unsatisfied therapeutic efficacy. A combined extract of black sticky rice and dill showed potential in preclinical state, so we hypothesized that it could provide clinical benefits. three-arm, randomized, placebo-controlled study was set up elucidate this issue. Both males females (18-80 years old) who had experienced transient ischemic attacks or strokes within last 5-10 days with an NIHSS score ≤ 7 received standard treatment were randomly assigned receive either a placebo capsule containing at dose 600 1200 mg per day. The safety parameters, movement control, degree disability assessed 1, 2, 6 weeks intervention, serum biomarkers mentioned time points, except 2 weeks. After week high-dose (1200 mg/day) group improved NIHSSS, VCAM1, MMP-9. S100β VCAM1 also 6, while low-dose (600 only exhibited VCAM1. Therefore, high developed supplement improves recovery by improving motor impairment reducing endothelial dysfunction inflammation.
