Major Depressive Disorder (MDD) significantly impacts millions worldwide, with limited success in achieving remission for many patients, leading to high disease burden and increased suicide risk. Psychotherapy antidepressants, although effective, do not provide relief all, prompting the search alternative treatments. Ketogenic diets have demonstrated positive effects on brain health. Our study aims investigate efficacy of ketogenic diet alleviating MDD symptoms, filling a critical gap psychiatric treatment options offering novel dietary approach potential mitigate enhance mental well-being. This phase randomized controlled trial will evaluate ten-week program dietitian counseling meal provision versus an intervention similar dietetic contact promoting healthy, insulin-lowering, non-ketogenic diet. The primary outcome is change Patient Health Questionnaire nine-item depression score. Secondary outcomes include cognitive affective mindfulness, self-efficacy, sleep, function, work social adjustment, various immunological, metabolic, microbiome markers at weeks 6 10. addresses by exploring diet's as intervention. Adhering to the ketogenic diet can reduce or stop seizures, even when other treatments fail, via mechanism(s) distinct from available therapies. These results have led interest in for treating conditions such as Alzheimer’s disease, depression and schizophrenia. Evidence points neuromodulator adenosine a key mechanism underlying therapeutic benefits of diet. Adenosine represents unique direct link among cell energy, neuronal activity, gene expression, receptors form functional heteromers with dopamine receptors. The importance dopaminergic system is established addiction, are challenges modulating directly. A mediator that could antagonize dopamine’s effects would be useful, due its function location. Studies report improves cognition, sociability, perseverative behaviors, might improve depression. Many translational opportunities based on diet/adenosine come fore, including autism spectrum disorder, painful conditions, range hyperdopaminergic disorders.

Schizoaffective disorder is a severe psychiatric condition characterized by mood disturbances and psychotic symptoms. Standard treatments, primarily pharmacological, often fail to control symptoms fully can lead significant metabolic side effects. Emerging evidence suggests that ketogenic therapy (KMT), also known as the diet, may offer powerful alternative conventional treatments for components resolve in patients with schizoaffective disorder. This case series investigates effects of KMT on two individuals diagnosed who pursued this due ineffectiveness treatments. Both presentations followed modified diet medical oversight. Symptom changes were assessed using validated tools, including Generalized Anxiety Disorder-7 (GAD-7), Depression Stress Scales (DASS-42), PTSD Checklist DSM-5 (PCL-5), Patient Health Questionnaire-9 (PHQ-9). experienced complete cessation improvements mood. Case 1, 17-year-old female, achieved full remission suicidal ideation, hallucinations, anxiety within 6 weeks, sustained at 24-week follow-up. 2, 32-year-old chronic months. Patients either deprescription or process time demonstrates disorder, leading functional recovery reported quality life extend beyond symptom standard care interventions.
