Abstract Background and Hypothesis Convergent evidence shows the presence of brain metabolic abnormalities in psychotic disorders. This study examined reductive stress energy metabolism people with disorders impaired or average range cognition. We hypothesized that global cognitive impairment would be associated greater dysregulation. Study Design Participants affective non-affective psychosis (n = 62) were administered MATRICS Consensus Cognitive Battery (MCCB) underwent a 31P-magnetic resonance spectroscopy scan at 4T. used cluster-analysis approach to identify 2 clusters participants without dysfunction. compared on redox balance stress, measured by ratio nicotinamide adenine dinucleotide (NAD+) its reduced form NADH, addition creatine kinase (CK) enzymatic activity pH. Results The mean (SD) age was 25.1 (6.3) years. NAD+/NADH differed between groups, lower ratio, suggesting more cluster (t −2.60, P .01). There also significant reduction CK −2.19, .03). Schizoaffective disorder is a severe psychiatric condition characterized by mood disturbances and psychotic symptoms. Standard treatments, primarily pharmacological, often fail to control symptoms fully can lead significant metabolic side effects. Emerging evidence suggests that ketogenic therapy (KMT), also known as the diet, may offer powerful alternative conventional treatments for components resolve in patients with schizoaffective disorder. This case series investigates effects of KMT on two individuals diagnosed who pursued this due ineffectiveness treatments. Both presentations followed modified diet medical oversight. Symptom changes were assessed using validated tools, including Generalized Anxiety Disorder-7 (GAD-7), Depression Stress Scales (DASS-42), PTSD Checklist DSM-5 (PCL-5), Patient Health Questionnaire-9 (PHQ-9). experienced complete cessation improvements mood. Case 1, 17-year-old female, achieved full remission suicidal ideation, hallucinations, anxiety within 6 weeks, sustained at 24-week follow-up. 2, 32-year-old chronic months. Patients either deprescription or process time demonstrates disorder, leading functional recovery reported quality life extend beyond symptom standard care interventions.

Abstract Introduction Inflammation, oxidative stress, and bioenergetic dysfunction are proposed underlying mechanisms of schizophrenia spectrum disorders (SSD) bipolar (BD), contributing to the largely untreated cognitive negative symptoms in these conditions. Ketone bodies may offer a therapeutic option for through their positive effects on aforementioned mechanisms. Exogenous ketones like ketone esters (KE) provide means quickly induce ketosis without dietary restrictions, though SSD BD have not yet been investigated. Here, we describe study protocol an ongoing triple-blind, randomized controlled crossover trial single ingestion KE signs BD. Methods A total 24 patients (12 SSD, 12 BD) receiving inpatient care at Amsterdam UMC will be included study. Patients ingest dose ((R)-3-hydroxybutyl-(R)-3-hydroxybutyrate deltaG® - dGK) isocaloric carbohydrate control with washout period three days between drinks. The primary outcome is change pre-pulse inhibition startle reflex (PPI) induced by dGK compared control. Secondary outcomes include resting-state EEG, P3B amplitude, performance, metabolic, immune, stress circadian rhythm parameters. Feasibility potential side also assessed. Discussion Our current offers valuable preliminary data patients. It can foundation future research into alleviating improving functional disorders.
