Abstract Background Medical deserts pose significant challenges to healthcare systems worldwide, leading unmet needs and exacerbated health issues, particularly in underserved regions. Methods This study aims characterise cancer care services the North-West region of Romania through lens medical desertification, employing a mixed-methods approach. Quantitative analysis secondary data from 2009 2022, along with qualitative interviews patients professionals, reveals disparities access across counties within region. Results While Cluj-Napoca exhibited robust infrastructure, areas like Satu-Mare Sălaj faced limitations accessing services, contributing desertification. The highlighted prevalence oncology, inadequate human resources, facility deficiencies, prolonged waiting times, high costs, socio-cultural barriers hindering care. Recommendations for improvement include revising treatment protocols, enhancing palliative care, implementing prevention strategies, promoting collaboration among digitalizing system. However, persist, including shortage oncology specialists, geographical prevalence, limited advanced modalities rural areas.
